Objective: To observe the efficacy of Sofosbuvir/Velpatasvir in the treatment of drug addicts and non-drug addicts with hepatitis C after antiviral therapy.Methods:A total of 80 patients with chronic hepatitis C who met the admission criteria in the outpatient and infection department of Qianxinan Prefecture people’s Hospital from July 1,2019 to June 1,2020 were collected.42 patients with hepatitis C were treated with methadone in Qianxinan compulsory detoxification center and Xingyi compulsory detoxification center,and the patients were divided into drug use group and non-drug use group,subgroup cirrhosis group and non-cirrhosis group.All the patients with hepatitis C were treated with Sofosbuvir/Velpatasvir regularly for 12 weeks.The liver function(ALT,AST,TBiL),HCV-RNA and liver cirrhosis related indexes(APRI,FIB-4)were measured at baseline,4 weeks after baseline,12 weeks,24 weeks and 36 weeks after baseline,and the difference of antiviral effect between drug addicts and non-drug addicts after antiviral treatment was evaluated.Results:1.After SOF/VEL intervention,the virus negative conversion rate of patients with hepatitis C in the non-drug group was significantly lower than that of patients with hepatitis C in the fourth week of treatment,and the difference was statistically significant(P <0.05).However,after 12 weeks of antiviral treatment,both groups obtained 100% persistent immune response,and there was no significant difference between the two groups(P > 0.05).2.After SOF/VEL intervention,ALT,AST,TBiL,APRI and FIB-4 were relieved in non-drug addicts and drug addicts with hepatitis C,but there was no significant difference in remission between the two groups,and there was no significant difference between the two groups(P > 0.05.The remission of ALT,AST and TBiL was observed in non-cirrhotic group and non-cirrhotic group,and the remission in non-cirrhotic group was significantly higher than that in non-cirrhotic group(P < 0.05).The APRI and FIB-4 of the two groups were relieved,but there was no significant difference between the two groups,so there was no significant difference between the two groups(P > 0.05).3.After SOF/VEL intervention,hepatitis B reactivation occurred in patients with HBs Ag positive in hepatitis C,and hepatitis B reactivation occurred in 2 patients.Conclusion:1.SOF/VEL directly interfered with the negative rate of HCV-RNA in hepatitis C at the4 th week of treatment,the remission in the non-drug group was more obvious than that in the drug group,and that in the subgroup was more obvious than that in the non-cirrhosis group,but the persistent viral response was obtained after 12 weeks of treatment;with the viral load turning negative,the liver function transaminase and liver function damage grade of the patients could be alleviated.2.After treatment,the indexes of FIB-4 and APRI in patients with hepatitis C decreased as the virus turned negative,indicating that radical cure of hepatitis C can not reverse liver fibrosis,but can slow down the process of liver cirrhosis.3.SOF/VEL treatment of hepatitis C may lead to potential hepatitis B virus reactivation.
