| Objectives:By analyzing the data of 120 patients with acute cerebral infarction in Taian Central Hospital,the relationship between cognitive impairment and carotid atherosclerosis(CAS),homocysteine(Hcy),hypersensitive C-reactive protein(hs-CRP),uric acid(UA)in patients with acute cerebral infarction was explored,which could provide reference for early intervention and treatment of cognitive impairment after cerebral infarction by clinicians.At the same time,it can lighten the burden of family and society.Methods:(1)A total of 120 first-episode acute cerebral infarction patients who were excluded according to the exclusion criteria and met the inclusion criteria were admitted to the Department of Neurology,Taian Central Hospital from December 2017 to October 2018.(2)We collected all the research objects of Mo CA,and the general information,such as sex,age,smoking history,hypertension history,diabetes history and so on.On the next morning after admission,fasting blood samples were taken to examine the levels of plasma Hcy,hs-CRP,UA,total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C)and low density lipoprotein cholesterol(LDL-C).All patients were examined by Color Doppler Ultrasound of Cervical Vessels.(3)According to the results of Mo CA evaluationg,120 patients were divided into cognitive impairment group(VCI group)and normal cognitive function group(without VCI group).(4)SPSS18.0 statistical software was used for data analysis,Chi-square test,independent sample T test,binary Logistic regression analysis,Spearman correlation analysis and other statistical methods were used for data analysis,P<0.05,the difference was statistically significant.Results:1.Comparison between the VCI group and the NVCI group:The two groups patients in gender,age,history of smoking and drinking were P>0.05,there was no significant difference between the two groups,The levels of Hcy,hs-CRP,UA and CAS in VCI group were significantly higher than NVCI group(P<0.05).There sults of regression analysis,Hcy(OR=1.494,95%CI=1.154-1.933,P=0.002),Hs-CRP(OR=2.649,95%CI=1.504-4.666,P=0.001),UA(OR=1.013,95%CI=1.002-1.024,P=0.020),CAS(OR=67.057,95%CI= 7.801-576.433,P=0.000).The mean scores of Mo CA and vasuospatial and executive,naming,attention,language,abstraction,memory,orientationin the VCI group were lower than those in the NVCI group.2.Spearman correlation analysis: The Hcy levels in VCI group were negatively correlated with vasuospatial and executive(r=-0.558,P<0.001),naming(r=-0.621,P<0.001),language(r=-0.404,P < 0.001),abstraction(r=-0.405,P < 0.001),memory(r=-0.356,P=0.001),Mo CA(r=-0.399,P<0.001),the difference is statistically significant.The UA levels in VCI group were negatively correlated with memory(r=-0.560,P<0.001),Mo CA(r=-0.225,P=0.039),the difference is statistically significant.The hs-CRP levels in VCI group were remarkable negative relevant relations with attention(r=-0.610,P<0.001),abstraction(r=-0.263,P=0.016),orientation(r=-0.657,P<0.001),Mo CA(r=-0.507,P<0.001),respectively,and the differences were statistically significant.Conclusions:1.Cognitive impairment is a common symptom in patients with acute cerebral infarction,which has a high incidence in patients with acute cerebral infarction.2.Hcy,hs-CRP,UA and CAS may be dangers for cognitive impairment after ACI.3.The levels of Hcy may be connected with dysfunction of vasuospatial,executive,language,naming,abstraction,memory.The UA levels may be related to impairment of memory.The hs-CRP levels maybe related to impairment of attention,abstraction,orientation.4.The determination of serum homocysteine,uric acid and hypersensitive CRP may have some predictive value for the degree of cognitive impairment in patients with cerebral infarction. |
