| BackgroundTuberculosis is a disease caused by Mycobacterium tuberculosis infection.According to the location of infection,it can be divided into pulmonary tuberculosis and lymph tuberculosis.China is a country with multiple tuberculosis.Drug treatment is mainly used for tuberculosis in clinic.However,the dosage of anti-tuberculosis drugs is large and the treatment course is long.During the treatment,due to the influence of individual constitution and other factors,the body cannot tolerate drugs and has low drug sensitivity.The specific response of the body to drugs and the toxic damage of drugs to the liver jointly induce anti-tuberculosis drug-induced liver injury(ATB-DILI).Anti-tuberculosis drug-induced liver injury(ATB-DILI)is mostly diffuse lesions,rapid progression,easy to complicated with liver failure,life-threatening.In order to avoid the disease of antituberculosis drug-induced liver injury reaching the degree of difficulty to control,timely discovery and symptomatic treatment is the primary method,which is of great help to the recovery of patients with liver injury.Computed tomography(CT)diagnosis is a common diagnostic method,which has the characteristics of objective imaging and convenient operation,and plays a positive role in clinical diagnosis and treatment and disease assessment.Most domestic scholars believe that the CT value of spleen is relatively fixed,and the liver / spleen ratio method is a common measurement method to measure the degree of fatty liver and liver density reduction.This study also tried to use this method to measure the reduction of liver density in anti-tuberculosis drug-induced liver injury.ObjectiveTo analyze the changes and clinical significance of CT measurement of liver and spleen in patients with anti-tuberculosis drug-induced liver injury(ATB-DILI).Materials and MethodsA total of 73 patients with anti-tuberculosis drug-induced liver injury(ATB-DILI)in the First Affiliated Hospital of Xinxiang Medical College from January 2020 to December2020 were selected as the anti-tuberculosis drug-induced liver injury(ATB-DILI)group,including 32 mild cases,25 moderate cases,and 16 severe cases.A 1:1 ratio was used to select 73 cases without liver dysfunction in the same period as the control group.CT diagnosis was performed in anti-tuberculosis drug-induced liver injury(ATB-DILI)group and healthy control group.The CT measurement values of the two groups were compared,including the CT value of liver,the difference between liver and spleen,and the difference between liver and spleen / Spleen ratio;The CT measurement value and abnormal rate of liver function of patients with different disease degree in anti-tuberculosis drug-induced liver injury(ATB-DILI)group were compared;The liver CT value and abnormal rate of liver function were compared before and after treatment in anti-tuberculosis drug-induced liver injury(ATB-DILI)group;The correlation between the changes of CT measurement of liver and spleen and abnormal liver function was analyzed.Results1.The liver CT value and liver/spleen ratio of the anti-tuberculosis drug-induced liver injury(ATB-DILI)group were lower than those of the control group,and the difference between liver and spleen of the anti-tuberculosis drug-induced liver injury(ATB-DILI)group was higher than that of the control group,the difference was statistically significant(P<0.05).2.In the anti-tuberculosis drug-induced liver injury(ATB-DILI)group,there were statistically significant differences in liver CT values,liver and spleen differences,and liver/spleen ratios in mild,moderate,and severe patients(P<0.05);the liver CT value and liver/spleen ratio of severe patients were lower than those of mild and moderate patients,and the difference between liver and spleen of severe patients was higher than that of mild and moderate patients,the difference was statistically significant(P<0.05).3.The abnormal liver function rate of severe patients with anti-tuberculosis dru g-induced liver injury(ATB-DILI)was 43.75%(7/16),higher than that of mild and moderate patients(6.25%,2/32;12.00%,3/25),the difference was statistically sig nificant(P<0.05).4.After treatment,liver CT values of mild,moderate,and severe patients were higher than those before treatment,and the difference was statistically significant(P<0.05).The abnormal liver function rate was lower than that before treatment,but there was no statistically significant difference among groups of mild,moderate,and severe patients(P>0.05).Spearman analysis showed that liver CT value and abnormal liver function rate were negatively correlated(r=-0.695,P<0.05).ConclusionsComputed tomography(CT)measurement values of anti-tuberculosis drug-induced liver injury(ATB-DILI)patients are abnormally expressed.The liver CT value and liver/spleen ratio are lower,and the difference between liver and spleen is higher.They are closely related to the severity of the disease and the rate of abnormal liver function,and the liver CT value and the rate of abnormal liver function are negatively correlated.Clinical diagnosis and treatment and prognosis evaluation can be implemented based on this. |
