ObjectivesThis study was to explore the pain nursing needs and coping strategies of patients with lumbar disc herniation(LDH)who received surgical treatment for the first time,and to understand the change trend of pain nursing needs and coping strategies in different stages,and to analyze the influencing factors,so as to effectively meet the pain nursing needs of patients and provide reference for the formulation and adjustment of phased pain nursing scheme.MethodsThis is a longitudinal study.A simple random sampling method was used to select 165 LDH patients who received surgical treatment for the first time in three Grade-A hospitals in Changsha from June to December2020.General information questionnaire,disease condition questionnaire,Numeric Rating Scale,pain nursing needs questionnaire and Coping Strategy Questionnaire were used to investigate the pain nursing needs and coping strategies on the first day of hospitalization,7 days and 1month after operation.SPSS 24.0 software was used for statistical analysis.The main statistical analysis methods were statistical description analysis,normality test,repeated measurement analysis of variance,correlation analysis and multiple linear regression analysis.Results(1)The results of the investigation on the needs of pain nursing for LDH patients(1)The total scores of pain nursing needs of LDH patients on the first day of hospitalization,7 days and 1 month after operation were(62.65±7.62),(62.20±11.37),(46.50±8.24)respectively.Compared with the scores of each dimension,the scores of physiological needs on the first day of hospitalization and the seventh day after operation were the highest,which were(18.63±2.83)and(16.54±3.71)respectively;the scores of social support needs on the first month after operation were the highest,which were(12.43±2.51).(2)By repeated measurement analysis of variance,there were significant differences in the total score of pain nursing needs,psychological,physiological,pain knowledge and social support needs of LDH patients at three time points(P<0.05).(3)By multiple linear regression analysis,the first day of hospitalization: education level,pain level and distraction strategy entered the regression model(F= 60.808,P<0.05);7 days after operation:occupational status,catastrophic strategy,distraction strategy and pain degree entered the regression model(F=6.905,P<0.05);One month after operation: catastrophic strategy,protruding segment and degree of pain entered the regression model(F=6.605,P<0.05).(2)The results of the investigation on coping strategies for LDH patients(1)The total scores of coping strategies of LDH patients on the first day of hospitalization,7 days after operation and 1 month after operation were(58.30±15.36),(54.92±14.79)and(44.52±10.01)respectively.Compared with the scores of each dimension,the distraction dimension had the highest scores at the three time points,which were(20.04±4.49),(21.59±4.98),(15.38±4.02).(2)By repeated measurement analysis of variance,there were significant differences in the total score of coping strategies,bravery,catastrophic,neglect,distraction and reinterpretation dimensions among LDH patients at three time points(P<0.05).(3)By multiple linear regression analysis,the first day of hospitalization: occupational status,medical insurance type,psychological needs,physiological needs and age entered the regression model(F=11.707,P<0.05);7 days after operation: psychological needs,self-management needs,analgesic methods and education entered the regression model(F=8.129,P<0.05);1 month after operation: age,marriage and education entered the regression model(F=5.948,P<0.05).Conclusions(1)Patients with LDH had higher demand for pain nursing on the first day of hospitalization and 7 days after operation.As time goes on,the demand for pain nursing after 1 month decreased significantly.Education level,pain degree and distraction were the influencing factors of pain nursing needs of LDH patients on the first day of hospitalization;occupational status,catastrophic strategies,distraction strategies and pain degree were the main influencing factors on the 7th day after operation;prominent segment,catastrophic strategies and pain degree were the main influencing factors on the 1st month after operation.(2)The coping strategies of LDH patients were mainly distraction at three time points,and the level of coping strategies gradually decreased with time.Age,occupational status,medical insurance type,physiological and psychological needs were the influencing factors of coping strategies of LDH patients on the first day of hospitalization;analgesia mode,education level,psychological and self-management needs were the main influencing factors on the 7th day after operation;age,marital and education level were the main influencing factors on the 1st month after operation.
