| Objectives:To survey the current condition of resourcefulness of patients with TC after surgery in Changsha 3A hospitals;To explore the correlation of intelligence level with psychological pain,core problems of quality of life,head & neck specific module;To analyze the influencing factors of resourcefulness in post-operative patients with TC in Changsha.Methods:By random sampling,309 TC patients who met the inclusion and exclusion criteria were selected randomly as the research objects after general surgery in three comprehensive tertiary first-class hospitals in Changsha from July 2020 to December 2020.The patients were investigated with self-designed general demo graphic data and hospital clinical rated at a questionnaire,resourcefulness scale,psychological pain screening tool scale,and head & neck specificity scale,which were designed with the tutor,were used simultaneously with Quality of Life Questionnaire Core for TC cancer patients.Spss24.0 statistical analysis:descriptive statistics was used for the general data of patients after thyroid cancer operation,(mean ± standard deviation)for the measurement data,frequency and composition ratio for the counting data;T-test and variance analysis were used for independent samples and multiple samples with normal distribution and homogeneous variance,otherwise,rank sum test was used Pearson Correlation Analysis was used to analyze the relationship between mental distress,core quality of life,head and neck specificity and resourcefulness level of cancer patients;Logistic regression analysis was used to determine the influencing factors of the thyroid cancer after operation.Results:(1)The total average point of resourcefulness of 309 patients after thyroid cancer surgery was(86.59 ± 20.65),which was in the middle level.Among them,87 patients(28.2%)had low intelligence level,and141 patients(45.6%)had medium resourcefulness level,there were 81patients(26.2%)with higher level of resourcefulness.(2)The psychological pain score of patients with thyroid cancer after operation was(3.87 ± 1.75),which was at a low level.Among them,patients with no,mild,moderate,severe and extreme psychological pain accounted for 1.6%,42.1%,50.2%,5.5% and 0.6% respectively.The positive rate of psychological pain was 56.96% in 174 patients with DT≥4.(3)The quality of life score of 309 patients after thyroid cancer surgery was(79.37 ± 4.12),which was in the middle level.The overall health score was(62.86 ± 23.67).In the functional domain,the social function score was the highest(86.62 ± 19.25),and the emotional function score was the lowest(79.26 ± 18.82),the highest score was fatigue(21.07 ± 18.66),the lowest score was diarrhea(5.61 ± 16.91),the total score of head & neck specific module was(42.39 ± 7.76),and the highest score of analgesic entry was(54.37 ± 49.89),the lowest score was feeding tube(1.62 ± 12.64).(4)The results of correlation analysis showed that the score of psychological pain degree of TC patients was negatively correlated with their total resourcefulness(r=-0.273,P<0.05).The total scores of core question of life and emotional function,cognitive function,social function and general health status in the universal scale of quality of life were positively correlated with the level of resourcefulness(r=0.261,0.239,0.131,0.184,0.333,P<0.05),while the scores of fatigue dimension and economic difficulty dimension were negatively correlated with the level of resourcefulness(r=-0.141,-0.164,P<0.05);The total points of head & neck specific module and pain dimension,swallowing dimension,sensation dimension,social disorder dimension,sexual function dimension,morbid feeling item,weight loss item in the head &neck specific module scale of quality of life were negatively correlated with resourcefulness level(r=-0.219,-0.161,-0.117,-0.155,-0.170,-0.116,-0.257,-0.129,P<0.05).(5)Univariate result analysis:marital status,educational level,work status,family origin,family per capita monthly income,payment method of medical expenses,and adjuvant therapy have statistical significance in the score of resourcefulness(P<0.05);The results of Logistic regression analysis showed that work status,monthly family income per capita,assistant treatment,psychological distress level,patients’ overall health status and Morbid state entered the regression model.Conclusions:(1)The resourcefulness level of thyroid cancer patients is above the average level,and individual resourcefulness is higher than social resourcefulness.(2)Through the analysis of this study,it was found that the level of mental pain,head and neck specific problems in patients with Changsha thyroid cancer were negatively correlated with the level of intelligence,while the core issues of quality of life were positively correlated with the level of intelligence,that is,the higher the level of psychological pain and the more specific symptoms,the lower the level of intelligence,the higher the quality of life of patients with higher level of intelligence.(3)The overall health status in the core issue of quality of life,morbid feeling,marital status,degree of psychological pain,adjuvant treatment and educational level in the specific module are the influencing factors of resourcefulness level. |
