ObjectiveEstablish a set of evaluation index system for expert patients with type 2 diabetes(T2DM)suitable for my country’s national conditions, and provide a reference for clinical training of T2 DM expert patients.MethodsThis study builds a theoretical framework based on self-efficacy theory, role recognition theory, peer support theory, and behavioral science theories. It uses literature research methods and semi-structured interview methods to preliminarily draw up the ability item pool for T2 DM expert patients, and establish it in combination with clinical conditions.T2 DM expert patient ability evaluation index system framework, using Delphi method to invite 20 experts to conduct two rounds of letter-inquiry preliminary screening ability evaluation indicators, using analytic hierarchy process to establish indicator weights, using SPSS 25.0 and Yaahp10.3 software for data processing and analysis, finalize the evaluation index system of T2 DM expert patient ability.Results1.In the two rounds of expert letter inquiries in this study,20 questionnaires were collected. The expert positive coefficients were 1.00,and the authority coefficients were 0.896 and 0.938,both of which were greater than 0.7.2.The Kendall coordination coefficient of the first round of correspondence is 0.549(P < 0.01);The coefficient of variation of five first-class indexes is between 0.05 and 0.17,and one index is modified; The coefficient of variation of 15 secondary indexes ranged from 0.05 to0.44,and one index was deleted and two indexes were revised. The coefficient of variation of 55 three-level indexes ranged from 0.05 to 0.48,and 6 indexes were revised and 10 indexes were deleted.3.The Kendall coordination coefficient of the second round of correspondence is 0.134(P < 0.01);The coefficient of variation of 5first-class indexes ranged from 0.00 to 0.15,that of 14 second-class indexes ranged from 0.00 to 0.15,and that of 45 third-class indexes ranged from 0.05 to 0.21,all of which were less than 0.25.One index was modified.4.Finalize the ability evaluation index system of T2 DM expert patients, including five first-class indexes(health information literacy,self-management ability, common decision-making ability, educational guidance ability and interpersonal communication ability),14second-level indicators(health information awareness, role management ability, ability to participate in decision-making evaluation, health education ability, etc.),and 45 third-level indicators. The weight coefficients of the first-class indicators obtained by AHP are health information literacy 0.2528,self-management ability 0.3689,common decision-making ability 0.0808,educational guidance ability 0.1111 and interpersonal communication ability 0.1864.The top three weight coefficients of secondary indicators are: active participation in disease or behavior management ability 0.1991,health information awareness0.1182,and emotional management ability 0.1095.The results of index consistency test are all between 0~0.0359 and less than 0.1.ConclusionConstructed a T2 DM expert-type patient ability evaluation index system, including 5 first-level indicators,14 second-level indicators,and45 third-level indicators.Experts are highly motivated and authoritative.Expert consultations are concentrated, coordinated, and itemized. The consensus of opinions is good, indicating that the results of the capability evaluation index system are worthy of credibility. The research method is reliable, and the hierarchical structure model has good consistency, which can provide a reference for the evaluation of the ability of T2 DM expert patients. In order to improve the self-care awareness of T2 DM patients and promote the self-health management of the whole people.
