| ObjectiveThe purpose of this study was to investigate the safety and efficacy of AngioJet mechanical thrombectomy versus manual aspiration thrombectomy combined with iliac vein angioplasty respectively for acute deep venous thrombosis(DVT)of lower extremity complicated with iliac vein compression.MethodsClinical data of 107 patients who were diagnosed as acute lower extremity deep venous thrombosis and voluntarily underwent interventional therapy from July 2016 to December 2020 were retrospectively analyzed.According to different treatment methods,patients were divided into two groups:68 patients treated with AngioJet Mechanical thrombectomy combined with stent implantation were called the AngioJet Group,and the left 39 patients treated with manual aspiration thrombectomy(MAT)combined with stent implantation were called the MAT Group.To evaluate the efficacy of the two treatment methods,the differences of improvement of clinical symptoms,duration of hospital stays,operation time and residual thrombosis between the two groups were compared and analyzed.The safety of the two treatments was evaluated by comparing the difference the total incidence of complications between the two groups.ResultsBaseline data of patients in the AngioJet group and the MAT group,including gender,age,disease time and type of deep vein thrombosis,were compared,and there were no significant statistical differences(P>0.05).AngioJet Group : clinical symptom remission rate:92.6%;duration of hospital stay: 8.35±1.26d;operation time:73.9±21.2min;64.7% of patients had no residual thrombosis or less than 5% residual thrombosis;the total incidence of complications: 29.4%.MAT Group:clinical symptom remission rate:69.2%;duration of hospital stay: 8.56±1.75d;operation time:71.6±22.3min;48.7% of patients had no residual thrombosis or less than 5% residual thrombosis;the total incidence of complications: 23.1%.The clinical symptom remission rate in the AngioJet Group were significantly higher than that in the MAT Group,and the residual thrombosis in the AngioJet Group were less than that in the MAT Group(P<0.05),but there was no statistical significant difference in the duration of hospital stays,operation time and the total incidence of complications between the AngioJet Group and the MAT Group(P>0.05).ConclusionFor acute DVT of lower extremity with IVCS,satisfactory therapeutic effect and safety can be achieved by adopting either AngioJet mechanical thrombectomy or manual aspiration thrombectomy,combined with iliac vein angioplasty.Compared with MAT,the improvement of clinical symptoms after AngioJet mechanical thrombectomy was higher,the residual thrombus was less,which means that the efficacy of AngioJet mechanical thrombectomy was better,while the safety of the two interventional therapies are comparable. |
