The Evaluation of a Predictive Model for the Vulnerable Plaque by Using Coronary-Cerebrovascular Integrated Computed Tomograph Angiography.Objective Using coronary-cerebrovascular integrated computed tomograph angiography(CTA),by analyzing the risk factors and following up the incidence of adverse events of plaques,a predication model of cardiovascular and cerebrovascular vulnerable atherosclerotic plaques was established and verified.Method Between September 2017 and December 2018,a total of 1015 consecutive symptomatic patients with suspected cardiovascular and/or cerebrovascular disease or asymptomatic population with cardiovascular risk factors were consecutively enrolled in this prospective study and underwent coronary-cerebrovascular integrated CTA.According to CT characteristics,they were divided into vulnerable plaque group,stable plaque group and normal group.Analyze the occurrence of cardiovascular and cerebrovascular plaques,and explore the correlation between the two parts.By analyzing differences in indicators such as age,gender,history of hypertension and diabetes,fasting blood glucose(FBG),triglyceride(TG),total cholesterol(TCH),high density lipoprotein(HDL),low density lipoprotein(LDL),C-reactive protein(CRP),Creatine Kinase(CK),Creatine Kinase-MB(CK-MB),to establish a predictive model for the vulnerable plaque by using coronary-cerebrovascular integrated CTA.A staging following up were collected,including the presence of angina pectoris,cardiovascular and cerebrovascular stroke,restenosis after stent or stent,and sudden death.Retrospective analysis of the incidence of major adverse cardio-cerebralvascular events(MACCE)of patients in vulnerable plaque group and stable plaque group at different ages and different onset times were performed to validate the predictive model of plaque vulnerability by using coronary-cerebrovascular integrated CTA.Results There were 290 cases(28.6%)in the vulnerable plaque group,178 cases(17.5%)in the stable plaque group,and 547 cases(53.9%)in the normal group.(1)There were 54 patients with coronary artery plaques only,28 patients with carotid and cerebrovascular plaques only,36 patients with both coronary and cerebrovascular plaques,and 82 patients without coronary or cerebrovascular plaques.Coronary plaques are correlated to cerebrovascular plaques(p <0.001).(2)Factors such as FBG(p <0.001),TG(p <0.01),LDL(p <0.01),HDL(p <0.01),CRP(p <0.001),hypertension(p <0.001)and diabetes(p <0.001)are the risk factors for the formation of vulnerable plaques.(3)The incidence of MACCE in the vulnerable plaque group was higher than that in the stable plaque group,and the long-term survival rate was lower than that in the stable plaque group.The difference was statistically significant(p <0.0001).Conclusions FBG,TG,LDL,HDL,CRP,hypertension and diabetes are risk factors for vulnerable plaques in the coronary and cerebrovascular.Vulnerable plaque is an independent risk factor for MACCE.coronary-cerebrovascular integrated CTA has an important value for early prediction of coronary-cerebrovascular disease.Preliminary Study on the Role of LncRNA HCG11 in the Stability of Coronary-Cerebrovascular Atherosclerotic PlaqueObjective Combining with long noncoding RNA HLA complex group 11(lncRNA HCG11),to develop a biomarker chip for vulnerable coronary-cerebrovascular plaques and clarify the rupture molecular mechanism of vulnerable coronary-cerebrovascular plaques.Method From 1015 cases,200 consecutive patients were selected.According to CT characteristics,they were divided into vulnerable plaque group,stable plaque group and normal group.Genetic methods were used to detect the plasma HCG11 expression of patients in different groups to analyze the difference of them and analyzing the correlation between plasma HCG11 and serum FBG expression of patients in vulnerable plaque groups.In vitro,explore the mechanism of high glucose(HG)-induced umbilical vein endothelial cells(HUVEC)inhibition of cells proliferation and tube formation,and the overexpression of HCG11 reverse the inhibitory effect of HG-induced HUVEC proliferation and tube formation,to investigate the role of plasma lncRNA HCG11 in plaque vulnerability.Results(1)Plasma HCG11 levels in the vulnerable plaque group(2.13±0.16)were higher than those in the stable plaque group(1.20±0.29)and the normal group(1.06±0.12),and the difference was statistically significant(p < 0.01).Plasma HCG11 levels in the stable plaque group were higher than those in normal group,but the difference was not statistically significant(p > 0.05).(2)There was a negative correlation between expression of plasma HCG11 and serum FBG in the vulnerable plaque group(R2 = 0.709,p < 0.0001).(3)It demonstrated in cell experiments that HCG11 was reduced in HG-treated HUVECs,and overexpression of HCG11 reversed the proliferation and tube formation inhibition of HG-treated HUVECs.Conclusions Plasma lncRNA HCG11 is overexpressed in patients with vulnerable plaques and it plays an important role in the mechanism of HG-induced endothelial cell damage.The lncRNA HCG11 detection chip is useful for the assessment of coronary-cerebrovascular atherosclerotic plaque stability.
