Objective:Lateral ankle sprain(LAS)is one of the most common lower limb sports injuries,which can progress to chronic ankle instability(CAI)if it is not treated properly.functional ankle instability(FAI)is a common type of CAI.In recent years,whole-body vibration training,as a new neuromuscular exercise training method,has attracted extensive attention from researchers and clinical personnel.However,there are few reports on whether whole-body vibration training in the FAI is better than conventional training,and whether there are advantages of whole-body vibration training combined with conventional training.Therefore,this study used randomized controlled trial to explore the effect of whole-body vibration combined with balance training on FAI compared with conventional training alone,in order to provide reference and basis for the treatment of FAI.Methods:Forty-five qualified FAI subjects were enrolled and randomly assigned according to 1:1:1 to whole body vibration group(WBV),balance training group(BT)and combined group(WBV+BT).WBV group was given vibration intervention for 6 weeks,3 times a week,30 min each time;BT group was given balance intervention for 6 weeks,3 times a week,30 min each time;The number of vibration training and balance training in the combined group were half as many as those in the other groups for a period of 6weeks,3 times a week,30 min each time.The tested items included the foot and ankle function assessed by Foot and Ankle Ability Measure(FAAM),dynamic balance ability evaluated by modified Star Excursion Balance Test(m SEBT),postural stability evaluated by Biodex balance test system,varus and valgus isokinetic muscle strength and joint position sense measured by isokinetic muscle strength test machine.SPSS24.0 for statistically analysis,the two-way repeated-measures ANOVA or nonparametric test were performed to compare the three interventions.P < 0.05 indicates a statistically significant difference.Results:(1)After intervention and follow-up,there was no significant difference in the improvement of FAAM-ADL among three groups(P >0.05).WBV+BT group was significantly better than WBV group and BT group in improving FAAM-sport(P <0.05),but there was no significant difference between WBV and BT group(P >0.05).(2)The overall stability index of the WBV+BT group was only significantly better than other groups after intervention(P <0.05),but there was no significant difference after follow-up(P >0.05);the antero-posterior stability index was both better than other groups after intervention and follow-up(P <0.05),There was no significant difference between BT group and WBV group(P >0.05);The medial-lateral stability index of BT group was only better than other groups after intervention(P <0.05),and there was no significant difference between BT group and WBV group(P >0.05).(3)The reach distance of anterior,posterolateral and posteromedial in WBV+BT group was significantly better than other groups(P <0.05),but there was no significant difference between WBV and BT group(P >0.05).(4)In WBV+BT group and WBV group,the improvement degree of varus and valgus peak torque was better than BT group,and there was no significant difference between WBV+BT group and WBV group(P >0.05).(5)The effect of WBV+BT group on the varus and valgus position sense was significantly better than other groups(P <0.05),and WBV group was better than BT group(P <0.05).Conclusions:(1)Six weeks of whole-body vibration training,balance training and combined training can improve FAI subjects’ postural control ability,dynamic balance ability,joint position sense and muscle strength,and can effectively increase the stability of ankle joint and improve the scores of foot and ankle function scale;(2)Six weeks of vibration training has a unique advantage in improving muscle strength,which is better than balance training;(3)The effect of six weeks combined training on the function improvement of FAI subjects was better than balance training and vibration training alone.
