| Objective:Through questionnaire surveys,it reveals the current status of caregiver burden of inpatient stroke patients, explores the influencing factors and action paths of caregiver burden of inpatient stroke patients, and provides a basis for targeted health guidance to caregivers of inpatient stroke patients. Methods:Taking caregivers of stroke patients who were treated in the neurology and rehabilitation department of two comprehensive tertiary hospitals in Cangzhou from April to November2020 as the research object, the convenience sampling method was adopted and 325 inpatient brains were selected in strict accordance with the inclusion and exclusion criteria.Caregivers of stroke patients, using the Pearlin stress process model as the theoretical framework, using the Zarit Care Burden Scale(Zarit-22),Concise Health Status Summary(SF-12),and Social Support Scale(SSRS)for hospitalized stroke patients The caregiver conducts a questionnaire survey and uses the Daily Activity Ability Rating Scale(ADL)to investigate the patient’s self-care ability. The database was established using SPSS 25.0 software, descriptive statistical analysis was used for care burden,t-test or analysis of variance was used for comparison of care burden with general data,Pearson bivariate correlation analysis was used for correlation, and multiple linear regression analysis was used for influencing factors of care burden.Use AMOS 22.0 software to build a structural equation model to explore the path of action of factors affecting the burden of care.Results:1.A total of 332 questionnaires were issued and 332 were returned. There were 325 valid questionnaires, with an effective rate of 97.89%.2.The current status of caregiver burden of hospitalized stroke patients. The total score of care burden is 26.63±5.92 points,of which the scores of personal burden, responsibility burden and other burden dimensions are 15.93±4.18 points,6.00±1.78 points, and 4.70±2.28 points,respectively.3.Influencing factors of caregiver burden of inpatient stroke patients.(1)Univariate analysis of general data: the caregiver’s age, religious belief, working status, relationship with patients, sleep status, average daily care time and combined chronic diseases had statistical significance in the score of care burden(P<0.05).(2)Correlation analysis between care burden and influencing factors: care burden has a significant negative correlation with patients’ self-care ability(r=-0.499,P < 0.01);it has a significant negative correlation with caregiver’s physical health, mental health, and social support Relationship(r=-0.271~-0.190,P<0.05).(3)Multivariate linear regression analysis of influencing factors of caregiver burden of hospitalized stroke patients: caregiver’s age, average daily care time, chronic disease, patient self-care ability, mental health, and social support are the influencing factors of care burden(P<0.05).(4)The path of influencing factors of the caregiver burden of inpatient stroke patients:the patient’s self-care ability and the caregiver’s social support directly affect the care burden;the patient’s self-care ability can indirectly affect the care burden through the physical health and mental health of the caregiver. The caregiver’s social support can affect the caregiver’s mental health and indirectly affect the care burden;the caregiver’s social support can also affect the caregiver’s mental health through the caregiver’s physical health, which in turn affects the care burden.Conclusions:1.Caregiver’s of hospitalized stroke patients have a burden of care, which is generally at a moderate level.2.There is a significant negative correlation between the patient’s self-care ability, the caregiver’s physical health, mental health, social support and the burden of care. The age of the caregiver, the average daily care time, combined chronic diseases, the patient’s self-care ability, the caregiver’s mental health, and social support are the influencing factors of the care burden. The self-care ability of patients and the social support of caregivers directly affect the burden of care;the self-care ability of patients can indirectly affect the burden of care through the physical and mental health of the caregiver. The caregiver’s social support can affect the caregiver’s mental health and thus affect the care burden;the caregiver’s social support can also affect the caregiver’s physical health through the caregiver’s mental health, which in turn affects the care burden.Improving the self-care ability of patients, promoting the physical and mental health of caregivers, and helping caregivers to establish a social support network will help reduce the burden of care. |
