Objective: The risk of death in people with schizophrenia is two to three times higher than in the general population,and life expectancy is shortened by nearly 20 years,with cardiovascular disease contributing to premature death in people with schizophrenia.In order to improve the quality of life of patients with schizophrenia and reduce cardiogenic death,this study aims to explore the related factors of cardiovascular abnormalities in patients with schizophrenia.Methods: Chronic schizophrenia patients with no history of cardiovascular disease were included in the study.Clinical data including general information,metabolic indexes,skin niacin response score and other clinical data were collected.Patients were treated with electrocardiogram,heart color ultrasound and Carotid intima media thickness.CIMT),pulse wave velocity,Ankle brachial index(ABI),heart rate variability.Firstly,the ECG of patients with schizophrenia was divided into groups with Minnesota coding,and the difference of clinical data was analyzed.The factors related to ECG abnormalities in schizophrenia were further explored by Spearman correlation,Logistic regression and ROC curve statistical methods.At the same time,GLS grouping was performed by using the normal threshold value of Global long-axis strain(GLS)in cardiac color Doppler ultrasound,and the same analysis method was used to explore the factors related to the early structural abnormalities of the heart in schizophrenia.Finally,the correlation between the equivalent of chlorpromazine and CIMT,ABI and heart rate variability was analyzed.Results: Severe ECG abnormalities in schizophrenia patients were correlated with chlorpromazine equivalent,but not with smoking,obesity,abnormal glucose and lipid metabolism,and skin niacin response score.The larger chlorpromazine equivalent was,the more likely severe ECG abnormalities were to occur.Abnormality of GLS was correlated with chlorpromazine equivalent and Body mass index(BMI),but not with smoking,obesity,abnormal glucose and lipid metabolism,or skin niacin response score.The higher chlorpromazine equivalent,the higher BMI,the more likely GLS abnormality was.There was a negative correlation between chlorpromazine equivalent and heart rate variability.Conclusion: The abnormalities of cardiac electrical activity and cardiac structure in patients with schizophrenia were correlated with the equivalent of chlorpromazine.It has some implications for clinical work.When schizophrenic patients take high dose of antipsychotic drugs,they should pay attention to their cardiac function state regardless of the risk factors of cardiovascular disease.
