Objective:By collecting clinical data of patients with AIDS and bloodstream infection,analyzing the characteristics of pathogenic bacteria,drug susceptibility characteristics and clinical prognosis-related factors of this group of patients,so as to provide help for the correct clinical use of antibiotics and prognosis.Methods:The clinical data of HIV-infected patients combined with BSI hospitalized in Hangzhou Xixi Hospital from June 2012 to December 2020 were collected retrospectively,The pathogenic bacteria were isolated from blood culture and drug sensitivity were tested.The patients were divided into survival group and death group based on the survival state at the end of this study.The laboratory results of the two groups were compared and the independent risk factors related to prognosis were screened by Logistic regression analysis.Results:The data of 214 patients were collected and 217 strains were isolated,including 77 Gram-positive bacteria(35.5%),55 Gram-negative bacteria(25.3%),62fungi(28.6%)and 23 acid-fast bacilli(10.6%).In terms of drug resistance,Gram-positive bacteria were not resistant to vancomycin and tegacycline.,and had good sensitivity to quinoloptin/dafoptin(resistance rate 5.1%),linezolid(resistance rate 2.2%)and nitrofurantoin(resistance rate 2.6%).Overall,Gram-negative bacteria maintained good sensitivity to ertapenem(resistance rate 9.3%),imipenem(resistance rate 10.7%)and piperacillin/tazobactam(resistance rate 11.7%).After adjustment for other factors,thrombocytopenia(Adjusted OR=0.144,95%CI 0.028-0.747)and septic shock(Adjusted OR=34.147,95%CI 2.568-454.105)were independent risk factors for death.Conclusion:Opportunistic pathogens are the most common pathogens in A IDS patients complicated with bloodstream infection.Gram-positive bacteria and Gram-negative bacteria are generally sensitive to high-grade antibiotics.For AI DS patients with bloodstream infection,the presence of septic shock and platel et<100×10~9/L may indicate a poor prognosis.
