| Objective: Acoustic neuroma is one of the most common primary tumors in neurosurgery.Tumor resection and nerve function preservation have always been a hot topic in neurosurgery.The purpose of this study is to investigate the application value of DCE-MRI in acoustic neuroma.The focus is to determine the blood supply of acoustic neuroma by DCE-MRI,and to analyze the value of semi-quantitative parameters of DCE-MRI in evaluating the microcirculation of acoustic neuroma,so as to provide objective basis for preoperative evaluation of acoustic neuroma.Methods: The subjects of this study were 16 patients with acoustic neuroma and8 patients without acoustic neuroma history were selected as control group.The 16 patients admitted were all routinely performed MRI plain scan and enhanced scan before operation,followed by DCE-MRI scan.They were grouped according to the enhancement characteristics of the lesions and divided into internal auditory canal group(6 cases)and intracranial group(10 cases));All patients in the control group were also scanned by DCE-MRI,that is,the control group(8 cases).The original DCE-MRI images were imported into Philips image processing workstation for post-processing and analysis.Time intensity curve(TIC)and semi quantitative parameters were obtained.Analyze the types of TIC curves in each group and perform statistical analysis on the semi-quantitative parameters of each group,and use the X2 test and Kruskal-wallis test to compare between groups.Results: In the control group,TIC curves of type Ⅰ,Ⅱ and Ⅲ were 8,0,and 0cases,respectively.There were 0,1,and 5 cases of TIC type I,II and III in the internal auditory canal group.There were 4,6,and 0 cases of TIC type I,II and III in the intracranial group.The comparison between the three groups shows clear differences(P<0.05);Comparing the intracranial group with the internal auditory canal group,Show a clear difference(P<0.05).SImax,TTP,MSI,WIR of the internal auditory canal group(267.2±51.7%,2.32±0.73 s,426±219.4%,1341.2±764.1 s-1)and the intracranial group(196±29.8%,4.22±0.41 s,232.3±73.2%,892.3±278.5 s-1)are all more than those of the control(23.2±5.3%,0.57±0.22 s,70.2±51.9%,114.3±62.8s-1),Show a clear difference(P<0.05);The SImax,TTP,WIR of the internal auditory canal group were compared with the intracranial group,and give no clear difference(P>0.05);The MSI of the intracranial group was lower than that of the internal auditory canal group,give a clear difference(P<0.05).Completion: 1.DCE-MRI is more favorable than conventional MRI plain scan and enhancement in observing the blood supply of acoustic neuroma,and the former can evaluate the blood supply of tumor more intuitively and clearly;2.The semi quantitative analysis of DCE-MRI showed that the tic and MSI have certain implication in distinguishing the blood supply of acoustic neuroma from the internal auditory canal or intracranial.3.DCE-MRI has certain clinical application value for preoperative assessment of the blood supply of acoustic neuroma. |
