A Study On The Soft Tissue Changes Of Skeletal Class Ⅲ With Orthodontic Orthognathic Treatment

Objective:To evaluate th e effect of orthodontic orthognathic tr eatment on facial soft tissue changes of patients with sk eletal Class III ma locclusion before and after treatment and to study the aesthetic score improvement of facial appearance before and aft er treatment by orthodo ntists and the layperson.Methods: 15 patients with skeletal Class III malocclusion treated by orthodontic orthognathic operation were selected.Frontal and 90 °right side photos were taken pre-and post-treatment.Each photo was div ided into upper,middle and lower 1 / 3 part of the face.30 evaluators were selected to make NRS(numerical rating sc ale)for each part of photos of the 30 patients.The CBCT films of 30 patients before and after operation were overlapped with the CBCT im ages.12 landmarks on the front were selected to analyze the normal soft tissu e of 18-line-distance indexes;Meanwhile,the lateral soft tissue of 11 indexes were an alyzed by Holdaway analysis method.Results: The NRS scores of full、lower 1 / 3、lower 2/ 3 part of frontal and90 °right side photos were higher than those before operation.Middle 1 / 3part of frontal and 90 °right side photos’ scores were lower.D-value of pre-post operation NRS arranged from low to high was: middle 1 / 3 of lateral photo,middle 1 / 3 of frontal photo,frontal、 lower 1 / 3 of frontal、lower 2 / 3 of front al photos,lateral、 lower 1 / 3 of lateral、 lower 2 / 3 of lateral photos.There was no significanc e in frontal 、lower 1 / 3 of frontal 、lower 2/ 3 of front al photos and lateral、 lower 1 / 3 of lateral 、 lower 2 / 3of lateral photos.Layperson have lower score than orthodontists in many parts.There was no significant difference in NRS between orthodontists and the layperson.In the frontal view,the nasal widt h,lip height,lower lip height became higher and chin height became smaller.In the lateral view,A-point convexity,H-angle,and depth of geniolabial sulcus increased,while Li-H line,upper lip thickness,and nasal co nvexity decreased.There was no difference in other measurement index.Conclusion: 1.Orthodontic orthognathic tre atment can improve both the frontal and lateral facial soft tissue of patients with skeletal Cl ass III,among which the lateral position,lower 1 / 3 lateral position and lower 2 /3 lateral position have the biggest improvement followed by the front al position,1 / 3 lower position and 2 / 3 lower position.2.There was no difference between orthod ontists and layperson in the D-values of NRS scores before and after orthodontic orthognathic treatment.3.Objective indexes were changed in the lower 2 / 3 of both sides...
