The purpose of surgical informed consent is to fully inform patients of the reasons for the operation,the proposed operation,the risks and benefits of accepting and rejecting the operation,and alternative treatment options,so that patients can make appropriate decisions.Surgical informed consent recognizes the autonomy and dignity of patients,it is also an opportunity for doctors to establish an open and trusting relationship with patients and to promote mutual understanding in the event of accidents in order to reduce unnecessary litigation.In clinical practice,preoperative conversation and informed consent form together constitute the clinical standard informed consent procedure,but in the specific implementation process,there were often insufficient implementation.Therefore,based on the surgical grading system,this study discussed the effect of the form of surgical informed consent(standard informed consent / preoperative conversation / informed consent)on the effect of patients’ consent under different surgical grades(second / third / fourth grade operation).The optimization effects of auxiliary material type(text / video)and risk quantification(with / without)on preoperative conversation and informed consent were further investigated.Finally,based on the integration of the research results,an optimization scheme for standard informed consent is proposed and verified.Study 1 based on the current clinical practice of inadequate implementation of surgical informed consent,the purpose of this study was to explore the effects of surgical grade(second / third / fourth grade operation)and the form of surgical informed consent(standard informed consent / preoperative conversation / informed consent form)on the effect of patients’ consent.The results showed that the grade of operation did not affect the effect of consent,the consent effect of informed consent form was the worst,and the consent effect of preoperative conversation was very close to that of standard informed consent,which indicated that under the premise of full implementation of preoperative conversation,the patient’s failure to read the informed consent form will not damage the effect of informed consent.The purpose of study 2 was to explore the effects of surgical grade(two / three / four surgery)and the type of auxiliary materials(text,video)on the consent effect of preoperative conversation and informed consent form.The results showed that in the form of preoperative conversation,the used of text auxiliary materials could effectively improve the effect of consent(experiment 2a),and in the form of informed consent form,the used of text / video auxiliary materials could effectively improve the effect of consent.And the effect of video auxiliary material was the best(experiment 2b).The purpose of study 3 was to explore the effects of surgical grade(grade 2 / 3 / 4)and risk quantification(before / after)on the effect of preoperative conversation and informed consent form.The results of experiment 3a and experiment 3b showed that the application of risk quantification in preoperative conversation and informed consent form can effectively improve the effect of informed consent.Understanding the risk probability information in the procedure of surgical informed consent was helpful to reduce patients’ risk perception and anxiety,and improve patients’ satisfaction with informed consent.Study 4 integratesd the experimental results of study 2 and study 3,and proposed an optimization scheme for the standard informed consent procedure,that was,standard preoperative conversation + text material + risk quantitative informed consent form.The results showed that the optimized surgical informed consent scheme was effective,which could significantly improve patients’ information comprehension,reduce patients’ anxiety and subjective perception of surgical risks.This study found that: informed consent form as the main form of implementation of informed consent will seriously damage the effect of informed consent,but if supplemented by video auxiliary materials can effectively make up for the shortcomings of informed consent form;in the form of preoperative conversation supplemented with text materials can improve the effect of informed consent to the greatest extent.The application of risk quantification in surgical informed consent could effectively reduce patients’ risk perception and anxiety and improve patient satisfaction;optimizing surgical informed consent(standard preoperative conversation + text materials + risk quantitative informed consent)could effectively improve the effect of clinical standard informed consent.
