| Aims:The advantages and disadvantages of radioactive seed implantation combined with chemical drugs in the treatment of pancreatic cancer were determined by meta-analysis.Method:Major Chinese and English databases,as well as clinical trial registries were searched with the keywords"pancreatic cancer"and"radioactive particles".By 2021-08-02,2 researchers with relevant knowledge background of statistics were selected to search according to pre-set inclusion and exclusion criteria.Revman5.3 was used for statistical analysis.Result:According to the inclusion and exclusion criteria,25 effective studies were included and 26 groups of effective data were extracted,with a total sample number of 1635 cases(811 cases in the radioactive particle combined chemotherapy group and 824 cases in the chemotherapy group).Compared with chemotherapeutic therapy alone,the combination of radioactive particles and chemotherapeutic therapy improved the clinical efficacy(CR+PR)by 1.82 times(RR=1.82>1,95%CI 1.59-2.07,and I~2=20%<50%,P=0.21>0.1),the improved clinical benefit rate(CBR)was 2.16 times(RR=2.16,95%CI 1.72-2.72,and I~2=0<50%,P=0.95>0.1),and the pain was reduced to 48%(RR=0.48<1,95%CI 0.38-0.61,and I~2=34%<50%,P=0.17>0.1),quality of life increased by 2.27 times(RR=2.27,95%CI 1.46~3.54,and I~2=34%<50%,P=0.17>0.1),12-month survival rate increased by 1.81 times(RR=1.81,95%CI 1.30-2.53,and I~2=38%<50%,P=0.17>0.1),in addition,there was no significant difference in the incidence of complications.Conclusion:1.Radionuclide interstitial implantation combined with chemotherapy is feasible and effective in the treatment of pancreatic cancer;2.Compared with chemotherapy alone,interstitial implantation of radioactive particles combined with chemotherapy can significantly improve the clinical remission rate,including partial and complete remission rate;3.Compared with chemotherapy alone for pancreatic cancer,interstitial implantation of radioactive particles combined with chemotherapy can significantly improve the clinical benefit rate(mainly manifested as postoperative pain relief,increased KPS score,weight change etc.);4.Compared with chemotherapy alone,interstitial implantation of radioactive particles combined with chemotherapy resulted in a significant reduction in postoperative pain;5.Compared with chemotherapy alone for pancreatic cancer,interstitial implantation of radioactive particles combined with chemotherapy significantly improved quality of life;6.Interstitial implantation of radioactive particles combined with chemotherapy improved 12-month survival compared with chemotherapy alone for pancreatic cancer;7.The results of the current study are not sufficient to demonstrate a significant difference between the two treatment schemes in the incidence of related complications;... |
