Objective:In this study,early glottic carcinoma was divided into different types according to the size of the tumor.The effect of hypothermic plasma radiofrequency ablation and open surgery on different types of tumors was compared.Whether the type of tumor has clinical significance in the selection of surgical method was discussed.The risk factors of postoperative tumor recurrence were further discussed.Methods:According to the inclusion criteria,109 laryngeal cancer patients undergoing surgical treatment in the Otolaryngology department of our hospital were selected strictly.By survival analysis,the disease-free survival rates of patients with different types and T stages of early glottic carcinoma who underwent hypothermic plasma radiofrequency ablation or open surgery were compared.Univariate and multivariate regression analysis were used to find out the risk factors for recurrence after hypothermic plasma radiofrequency ablation surgery.Results:There was no significant difference in disease-free survival rate between the two types of operations.With the increase of tumor size type and T stage,the disease-free survival rate decreased significantly in both groups.In addition to T stage and tumor type,smoking and subglottic involvement were risk factors for recurrence of early glottic carcinoma of type A.Conclusion:1.Open surgery is more common in early glottic carcinomas with higher tumor types,but there is no evidence that it is more effective than hypothermic plasma radiofrequency ablation.2.There was no significant difference in prognosis between the two methods(open surgery and hypothermic plasma radiofrequency ablation)for early glottic carcinoma with same type or T stage.3.Tumor type and T stage were inversely proportional to prognosis,that is,patients with higher tumor type /T stage had worse prognosis.4.Smoke,tumor type and T stage were independent risk factors for recurrence after hypothermic plasma radiofrequency ablation therapy.5.It is of certain clinical significance to classify early glottic carcinoma according to tumor size.
