| Objective:The purpose of this study is to construct a bowel intervention program based on NBD score,evaluate the safety and efficacy of this program for patients with neurogenic bowel dysfunction after spinal cord injury,and explore the influence of this program on bowel function,neuromuscular function and quality of life of patients with spinal cord injury.This study provides a new intervention program for the treatment of neurogenic bowel dysfunction after spinal cord injury.Methods:A total of 100 postoperative patients who were hospitalized at the Rehabilitation Department of a tertiary hospital in Changchun from December 1,2020 to November31,2021 were selected as the study subjects.The subjects were randomly divided into the intervention group and the control group by block randomization.The patients in the intervention group received nursing intervention based on NBD score,and the patients in the control group received bowel intervention program for 3 weeks.After the intervention,the NBD score of the patients was assessed weekly,and the intervention program was adjusted according to the change of the NBD score.Bowel function indicators(Wexner fecal incontinence score,stool control score,defecation time,Bristol stool characteristics,drug dependence),pelvic floor surface electromyography and Quality of life scale were collected to evaluate the effect on patients at three time points including before,after,and 1 month after nursing intervention.Result:1.After implementing bowel intervention program,the patients in the intervention group were evaluated on incidence of bloating,spinal cord independence assessment III-bowel management,defecation time,Bristol fecal character classification,drug dependence,fecal incontinence score and other bowel function indicators.After intervention and one month after intervention,they were better than the control group,and the difference was statistically significant(P <0.05)except for fecal incontinence score.2.The results of intra-group comparison showed that after the intervention and 1month after the intervention,the bowel function indicators of the patients in two groups were improved,and the difference was statistically significant(P <0.05).3.The results of pelvic floor surface electromyography showed that after intervention,the signal amplitudes of the intervention group were higher than those of the control group in the fast contraction phase,tension contraction phase,and endurance contraction phase,and the difference was statistically significant(P <0.05).4.The implementation of the program significantly improved the quality of life of the patients.The total score of the quality of life scale and the scores of each field of the intervention group were higher than those of the control group,and the differences were statistically significant(P <0.05)except for social field.Conclusions:1.The intervention program based on NBD score can effectively improve the bowel function of patients with neurogenic intestinal dysfunction after spinal cord injury.2.The intervention programs based on NBD score can improve the quality of life of patients with neurogenic bowel dysfunction after spinal cord injury. |
