Objective:To explore the relationship between Helicobacter pylori infection, specific allergen and chronic spontaneous urticaria,to provide evidence for its participation in the pathogenesis of chronic spontaneous urticaria,and to provide reference for clinical diagnosis,treatment and intervention of chronic spontaneous urticaria.Methods:A total of 128 patients who met the diagnostic criteria of chronic spontaneous urticaria in the Department of Dermatology of the second Hospital of Jilin University from September 2020 to December 2021 were collected.Helicobacter pylori and serum specific allergen Ig E (specific Ig E,sIgE)were detected in all patients,and 50 healthy persons in the physical examination center in the same period were randomly selected as healthy controls for Helicobacter pylori detection.Helicobacter pylori was detected by urea[14C]breath test box of China Nuclear Hedway Company,and specific allergen was detected by Allergy Screen Test Kit of MEDIWISS company in Germany.The clinical data(sex,age,severity,lesion distribution,treatment effect,etc.)of patients were collected and sorted in Excel table.Statistical software SPSS26.0 was used to analyze the data.The adoption rate of counting data(%)indicates that theχ~2test is used between groups.The metrological data in accordance with the normal distribution were expressed by mean±standard deviation((?)±s),t-test was used,and non-conformance was expressed by median and quartile spacing(M,Q),and rank sum test was used.P<0.05 means that the difference is statistically significant.Results:This study collected a total of 128 CSU patients from the Department of Dermatology,the second Hospital of Jilin University from September2020 to December 2021,aged from 16 to 72 years old,including 36 male CSU patients(28.1%of the total,with an average age of 41.53±12.97years)and 92 female CSU patients(71.9%of the total,with an average age of 39.66±13.24 years).The sex ratio of male to female CSU patients is about 2:5.Among the 128patients with CSU,48 cases were Hp positive,the positive rate was 37.5%,and 50 cases in the healthy control group,including 11 cases of Hp(+)and 39 cases of Hp(-),with a positive rate of 22%.The positive rate of Hp in CSU group was higher than that in healthy control group,and the difference was statistically significant(χ~2=3.898,P<0.05).Among the 36 male patients,13 were Hp positive,with a positive rate of 36.1%,92 female patients,and 35 Hp positive cases,with a positive rate of 38.0%.There was no significant difference between the two groups(P>0.05).The total positive rate of serum specific allergen Ig E,49 was 38.28%in patients with CSU.In the end,10 of the Hp-positive CSU patients were sIgE-positive,and the positive rate was 20.8%.39 of the sIgE-negative CSU patients were positive,with a positive rate of 48.8%.Compared with the positive rate of sIgE in Hp-negative and Hp-positive CSU patients,the positive rate of sIgE in Hp-negative CSU patients was higher,and the difference was statistically significant(χ~2=9.896,P<0.05).Of the 128 CSU patients,49 were sIgE positive.The top 5 allergens in Hp positive CSU patients were Candida/Penicillium/Cladosporium/ Alternaria/Aspergillus Niger,peach/apple/mango/litchi/strawberry,dwarf ragweed,Platanus/ash,wheat/buckwheat.The top five allergens in Hp-negative CSU patients were humulus,Dermatophagoides farinae,chicken protein,household dust mite,juniper/birch,and the top five allergens in overall CSU patients were humulus,Dermatophagoides farinae,chicken protein,ragweed,Candida/Penicillium/mycosporum/Alternaria/Aspergillus Niger.Among the 49 sIgE positive cases,26(53.06%)were allergen 1 positive,11(22.45%)were allergen 2 positive,7(14.29%)were 3 positive,2(4.08%)were 4 positive,and 3(6.12%)were 5 or more positive.There was no significant difference between the two groups.The severity of the disease was evaluated in all the patients with CSU.Among the Hp positive CSU patients,1 case was mild,13 moderate and34 severe,while in the Hp negative CSU patients,5 mild,33 moderate and42 severe.The condition of Hp positive CSU patients was more severe than that of Hp negative CSU patients,and the difference was statistically significant(Z=-2.100,P<0.05).Conclusions:1.There is a great difference in the sex ratio of CSU patients in Jilin Province,and the proportion of females is significantly higher than that of males.There was a significant difference in the positive rate of Hp between CSU patients and healthy people,but there was no significant correlation between gender and Hp infection.2.There was a significant difference in the positive rate of sIgE between Hp positive CSU patients and Hp negative CSU patients in Jilin Province.The positive rate of sIgE in Hp positive CSU patients was lower than that in Hp negative CSU patients.3.The common allergens in patients with CSU in Jilin Province are:humus,Dermatophagoides farinae,chicken protein,dwarf ragweed,Candida/Penicillium/Cladosporium/Alternaria/Aspergillus Niger;the common allergens in Hp positive CSU patients are Candida/Penicillium/ Cladosporium/Aspergillus Niger,peach/apple/mango/litchi/strawberry,dwarf ragweed,raspberry/white wax,wheat/buckwheat.The common CSU patients with negative Hp are:humilis,Dermatophagoides farinae,chicken protein,household dust mite,juniper/birch.The types of allergens in Hp positive CSU patients were significantly different from those in common ones,while those in Hp negative patients were consistent with most of the results.4.Many kinds of allergens can be detected in CSU patients in Jilin Province,and there is no significant difference in the proportion of different types of allergens detected between Hp positive and Hp negative CSU patients.5.In Jilin Province,the severity of Hp-positive CSU was higher than that of Hp-negative CSU patients,and the proportion of severe patients was 70.8%.There was a significant difference between the two groups.
