Objective To study the changes of p H of peri-implant crevicular fluid(PICF)and clinical indicators in patients with chronic periodontitis with loading time,provide a simpler and faster clinical diagnosis for the early diagnosis of peri-implantitis status.In order to judge the occurrence of peri-implant inflammation from the clinical detection level and take timely intervention measures,provide clinical reference for primary prevention of perinatal disease.Methods A total of 77 patients who received implant restoration from October 2014 to October 2021 in the Department of Periodontal Mucosa,Affiliated Stomatological Hospital of Anhui Medical University were collected as the experimental group,and were grouped according to the loading time of intraoral implants.A total of 401 sites were detected.During the detection period,a total of 12 patients diagnosed with periodontal health were included as the control group,and 60 sites were detected.First,the basic information(name,gender,age,etc.),intraoral implant-related informations(including implant system,implant specifications,implant loading years,etc.),intraoral data and imaging studies of the experimental group that met the inclusion criteria were saved.According to the implants loading time of the experimental group,they were divided into 6 groups: 1 month,6 months,1 year,3years,5 years and 7years.Then,the p H of the peri-implant gingival crevicular fluid and the clinical parameters around the implant,including peri-implant probing depth(PPD),modification sulcus bleeding index(m BI),and modification plaque index(m PLI)and bleeding on probing(BOP)were detected and recorded;the peri-implant health and inflammation status of each group in the experimental group were judged and recorded according to the relevant indicators.For analysis,the p H,PPD/PD,m PLI,and m BI of the control group and the experimental group were compared and analyzed,and IBM SPSS 26.0 was used for statistical analysis.Continuous variables were described by the mean ± standard deviation( ± s).One-way ANOVA analysis of variance and Tukey’s method were used to compare the mean values between groups,and P< 0.05 was considered to be statistically significant.To verify the changes of p H of peri-implant gingival crevicular fluid and clinical parameters with loading time in patients with chronic periodontitis,and to provide clinical guidance for the primary prevention of peri-implant diseases.Results 1.The results of the study showed that with the change of loading time,The changes of the p H of PICF,PPD,m PLI,and m BI were statistically significant(P<0.001).2.The p H of PICF at the initial stage of loading is close to the healthy natural periodontal p H value.When the loading time exceeds 3 years,the p H of PICF tends to be alkaline and around 7.0 can assist in the diagnosis of peri-implant mucositis.3.The peri-implant probing depth(PPD)is significantly higher than the healthy periodontal probing depth(PD).The PD of natural teeth is about 2.0mm,and the PPD is 3-5mm.When the peri-implant probing depth(PPD)≥ 5mm,it indicates the occurrence of peri-implant mucositis.4.The peri-implant modified plaque index(m PLI)increased with the increase of loading time,and when m PLI increased significantly,it indicated that peri-implant plaque control should take further interventions.5.There was no site diagnosed as peri-implantitis in the experimental group.The incidence of peri-implant mucositis is 25%-55%,suggesting that regular review and maintenance in patients with chronic periodontitis can guarantee a higher implant survival rate.Conclusion In this study,the changes of the p H of PICF,peri-implant probing depth,modified plaque index,and modified bleeding index with loading time in patients with chronic periodontitis were observed,suggesting that the p H of PICF,PPD,m PLI and m BI changes can be used to assist in diagnosing the occurrence trend of peri-implant mucositis in clinical work,instructing clinicians to take timely measures for maintenance treatment and personalised oral hygiene guidance for patients,appropriately shortening the review interval,and preventing the occurrence of peri-implantitis.
