Efficacy Of H-UPPP Combined With Airvance Tongue Base Suspension In The Treatment Of Moderate To Severe OSA In Adults

Objective To investigate the efficacy of H-UPPP combined with Airvance tongue base suspension in the treatment of moderate to severe OSA in adults.Methods A total of 38 moderate to severe OSA patients（20 moderate and 18 severe）admitted to High-tech Hospital of The First Affiliated Hospital of Anhui Medical University from October 2019 to November 2020 were selected.After preoperative location diagnosis（DISE）was accompanied by oropharyngeal and tongue base plane obstruction,H-UPPP combined with Airvance tongue base suspension was performed.Follow-up at 1 year after surgery: PSG,Blood pressure,lipids,Epworth Sleepiness Scale（ESS）,Brief Health Questionnaire（SF-36）.Results 5 cases（13.2%）were cured,15 cases（39.5%）had obvious effect,8 cases（21.0%）were effective,and 10 cases（26.3%）had no effect.The total effective rate was73.7%.There were statistically significant differences in blood pressure,AHI,LSa O2,MSa O2,CT90,LDL-C,TG,ESS and SF-36 before and after surgery（P<0.05）;There was no statistical difference in BMI,TC,HDL-C;Compared with the ineffective group,the partial blocking rate of velopharyngeal and tongue root plane was higher in the improved group（cured + obvious effective + effective）,and the complete blocking rate was lower in the former group before surgery,and the difference between the two groups was statistically significant（P < 0.05）.There was no difference in preoperative obstruction of oropharynx and epiglottis between the two groups（P > 0.05）.Conclusion1.H-UPPP combined with Airvance Tongue-base suspension can effectively treat moderate to severe OSA in adults;To improve the blood pressure and lipids of moderate and severe OSA patients;And improve the sleep and life quality of patients.2.DISE can effectively evaluate the stenosis of the upper airway in real time,which has good clinical guiding value for the surgical indication and effective implementation of H-UPPP combined with Airvance Tongue base suspension.
