| Objective: To compare and analyze the clinical effects of laparoscopic radical cystectomy via extraperitoneal approach and traditional abdominal surgery combined with orthotopic neobladder.Methods: The clinical data of 42 male patients with bladder cancer who underwent laparoscopic radical cystectomy combined with orthotopic ileal neobladder in our center from May 2018 to August 2020 were retrospectively analyzed.According to the operation methods,they were divided into extraperitoneal laparoscopic radical cystectomy(extra peritoneal radical cystectomy,ELRC)group and transtraperitoneal laparoscopic radical cystectomy(tlrc)group,the basic data,perioperative period,oncology and complications of the two groups were collected and analyzed.Results: A total of 42 patients were included in the final analysis,and the average follow-up period was 15 months.Compared with patients in TLRC group,patients in ELRC group have shorter postoperative ventilation time and hospitalization time,and fewer early gastrointestinal complications.In addition,there was no significant difference in tumor recurrence,tumor specificity,overall survival rate and recurrence pattern between the two groups.Conclusion: In this study,we found that ELRC is a safe,efficient and repeatable operation method,and compared with the traditional TLRC operation method,ELRC operation method can reduce postoperative ventilation and hospitalization time,and reduce the occurrence of gastrointestinal complications.However,these advantages need long-term follow-up to be further confirmed. |
