Effect Of Left Colonic Artery Preservation On Near And Medium-term Follow-up Evaluation Of Laparoscopic Radical Resection Of Rectal Cancer

Objective To investigate the short and medium-term follow-up effect of left colonic artery preservation on laparoscopic radical resection of rectal cancer.Methods The data of 95 patients with rectal cancer who underwent laparoscopic radical resection of rectal cancer in the First Affiliated Hospital of Bengbu Medical College from January 2017 to June 2019 were retrospectively analyzed.According to whether the left colonic artery was preserved during the operation,48 patients with preserved left colonic artery during the operation were included in the retention group,and 47 patients without preserved left colonic artery during the operation were included in the non retention group.The patients were followed up for 6 months to 34 months.The operation related indexes,gastrointestinal function recovery,complications,12,24,and 34 months survival rate between the two groups.Results(1)There was no significant difference in gender,age,body mass index(BMI),distance between tumor and anal margin,tumor diameter and TNM stage between the two groups(P > 0.05).(2)The operation time,the first anal exhaust time,the recovery time of bowel sounds and the first eating time in the retention group were shorter than those in the non retention group,the total amount of intraoperative blood loss was less than those in the non retention group,and the number of lymph node dissections in tumor was less than those in the non retention group(P < 0.05).(3)The postoperative complication rate in the retention group was 8.33%,which was lower than 17.02% in the non retention group,but there was no significant difference(P > 0.05).(4)The 3-year overall survival rate in the retention group was 95.83%higher than 87.23% in the non retention group(P > 0.05)..Conclusion The preservation of left colonic artery in laparoscopic radical resection of rectal cancer can shorten the operation time,improve the total amount of intraoperative bleeding,benefit the recovery of gastrointestinal function,have less complications,have a high near and medium-term survival rate,and have a positive effect on improving the quality of life after operation.
