Objective:The purpose of this study was to investigate the relationship between preoperative C-reactive protein/albumin ratio(CRP/ALB),platelet count and C-reactive protein multiplier(P-CRP)and clinical prognosis in patients with non-small cell lung cancer.Methods:1.From January 2015 to December 2015,284 patients who underwent surgery for lung cancer in the First Affiliated Hospital of Bengbu Medical College were selected.According to their 3-year and 5-year survival conditions,ROC curves were drawn and the best cut-off value was determined.2.According to the cut-off value,the patients were divided into high CRP/ALB group,low CRP/ALB group,high P-CRP group and low P-CRP group.3.Survival curves were described by Kaplan-Meier method and compared by Logrank test.The univariate and multivariate analyses were performed with the Cox proportional hazard model to evaluate the prognostic factors.Results:1.With follow-up for 3 years for the survival end,the areas under the ROC curve of CRP/ALB and P-CRP were 0.537 and 0.533,respectively,and the cut-off values were0.094 and 0.933,respectively,and they were divided into high CRP/ALB group(>0.094),low CRP/ALB group(<0.094);high P-CRP group(>0.933),low P-CRP group(<0.933).Clinicopathological data showed that preoperative CRP/ALB and P-CRP levels were significantly correlated with TNM staging.The Kaplan-Meier survival curve showed that the 3-year survival rate of NSCLC patients had no significant difference between the high CRP/ALB group and the low CRP/ALB group;there was no significant difference between the high P-CRP group and the low P-CRP group;both indexes were high Compared with the group with low two indicators or the two indicators are inconsistent,no significant difference was found(P>0.05).2.With follow-up for 5 years for the survival end,the areas under the ROC curve of CRP/ALB and P-CRP were 0.688 and 0.672,respectively,and the cut-off values were0.070 and 0.659,respectively.This group was divided into high CRP/ALB group(>0.070),low CRP/ALB group(<0.070);high P-CRP group(>0.659),low P-CR group(<0.659).Clinicopathological data showed that preoperative CRP/ALB and P-CRP levels were significantly correlated with lymph node metastasis and TNM staging.The KaplanMeier survival curve showed that in the 5-year survival rate of patients,the high CRP/ALB group was significantly lower than the low CRP/ALB group,and the high PCRP group was significantly lower than the low P-CRP group.The group with both high indexes was lower than the group with both low indexes or the two indexes were inconsistent,and the difference was statistically significant(P<0.05).3.Univariate Cox regression analysis showed that CRP/ALB,P-CRP,age,smoking history,lymph node metastasis and TNM stage were the risk factors affecting the survival time after surgery(P<0.05).Multivariate Cox regression analysis showed that CRP/ALB,age,smoking history,lymph node metastasis and TNM stage were an independent risk factor affecting the survival time after surgery(P<0.05).Conclusion:1.Preoperative CRP/ALB,P-CRP levels may be related to poor overall survival.2.Preoperative CRP/ALB,P-CRP level prediction long-term prognosis is greater than recent.3.Preoperative CRP/ALB levels can more accurately reflect the overall survival of NSCLC patients than P-CRP.4.Preoperative CRP/ALB levels may be independent risk factors affecting the overall survival of NSCLC patients.
