BackgroundAcute myocardial infarction(AMI)is a common Acute and critical disease in cardiology,with rapid onset,rapid progression,poor prognosis and potentially huge risk.It was previously believed that most patients with AMI had obvious obstructive lesions.With the increase of AMI patients and the development and popularization of Coronary arteriography(CAG),it was found that about 10% of PATIENTS with AMI did not have obvious stenosis in CAG,that is,stenosis < 50%.According to 2016 European Society of Cardiology(ESC)opinion,Patients with this type of AMI were identified as coronary artery infarction with non-obstructive Myocardial infarction(MINOCA).The analysis of gender differences in clinical characteristics,treatment and prognosis of AMI patients has been confirmed in a large number of previous reports.MINOCA is an important classification of AMI,and relevant studies show that MINOCA is mostly seen in young female patients.However,there are few studies on gender differences in clinical characteristics of MINOCA patients.This study explored gender differences in clinical characteristics of MINOCA patients,and further analyzed the influencing factors of adverse cardiovascular events in MINOCA patients,so as to improve clinicians’ understanding of MINOCA patients.ObjectiveThis study explored gender differences in clinical characteristics of MINOCA patients,and further analyzed the influencing factors of adverse cardiovascular events in MINOCA patients,so as to improve clinicians’ understanding of MINOCA patients.MethodsIn this study,92 patients diagnosed with MINOCA and underwent CAG examination in the department of Cardiology,The Third Affiliated Hospital of Xinxiang Medical College from January 2018 to June 2021 were selected,including 55 male patients and 37 female patients.They were divided into two groups according to gender.Age,body mass index,and previous concomitant diseases(such as hypertension,hyperlipidemia,angina pectoris,cerebrovascular disease,diabetes,etc.)of patients in the two groups were collected.On admission,vital signs(heart rate,systolic blood pressure,diastolic blood pressure),blood lipid indicators(fasting day after admission),fasting blood glucose(FPG),peak value of creatine kinase isoenzyme(CK-MB),peak value of troponin I(c Tn I),D-dimer,N-terminal precursor brain natriuretic peptide(NT-pro BNP),C-reactive protein,Killip grading,coronary angiography Results(plaque,spasm,etc.)and the use of drugs during hospitalization(dual antiplatelet therapy,statins,nitrates,heparin,etc.);At the same time,the incidence of adverse cardiovascular events(MACE)such as nosocomial heart failure,cardiogenic shock,arrhythmia and recurrent angina pectoris in MINOCA patients was recorded.The general clinical data,auxiliary examination and test results of MINOCA patients with different genders were compared.Finally,multivariate Logistic regression was used to analyze the influencing factors of nosocomial adverse cardiovascular events in MINOCA patients.Results1.Female MINOCA patients were older than male patients,and their body mass index was lower than male patients,the proportion of hypertension and diabetes was higher,and the proportion of smoking was lower than male patients,with statistically significant differences.All P<0.05.2.The peak c Tn I level of female MINOCA patients during hospitalization was lower than that of male patients,and the difference was statistically significant.P<0.05.3.Age(OR = 1.131,95%CI: 1.024~1.250,P = 0.015);Diabetes mellitus(OR =5.350,95%CI: 1.213 ~ 23.587,P = 0.027)was significantly associated with adverse cardiovascular events in MINOCA patients.Conclusion1.The onset age of MINOCA patients in females was later than that in males,and the proportion of MINOCA patients with cardiovascular risk factors hypertension and diabetes was higher.2.Age and diabetes mellitus were independent risk factors for adverse cardiovascular events during hospitalization in MINOCA patients.
