| Background As the most common type of stroke,cerebral infarction has become a major disease burden in China due to its high morbidity and mortality.Inflammatory response is one of the important pathophysiological mechanisms of cerebral infarction,which is closely related to the severity and clinical outcome of cerebral infarction.Inflammatory response as a new therapeutic target has great potential.The treatment of inflammatory response in the acute stage of cerebral infarction can improve the neurological deficit of patients.As a new medical gas,Hydrogen exerts neuroprotective effects by inhibiting inflammatory responses.Objective To evaluate the effect of inhaled Hydrogen therapy on serum IL-6 and TNF-αlevels,NIHSS score,Barthel index and m RS score in patients with acute cerebral infarction.Methods The patients with acute cerebral infarction treated in our hospital from June 2020 to April 2021 were collected.The patients were randomly assigned to the control group and the observation group according to sex,TOAST criteria and thrombolysis or not.The control group was given routine treatment,and the observation group was given H2inhalation with concentration of about 3%and flow rate of 500 ml/min on the basis of routine treatment,1 hour at a time,twice a day,with an interval of 6-8 hours for 7 days.The serum levels of IL-6 and TNF-αlevels were compared between the two groups before and after treatment,and the NIHSS score,Barthel Index and m RS score were also compared at before and after treatment,14 days,30 days and 90 days after treatment,and the incidence of adverse reactions were compared between the two groups.Results1.Comparison of baseline data:there was no significant difference between the two groups in the time from onset to admission,past medical history and the distribution of anterior and posterior circulation infarction(P>0.05).2.The level of IL-6 and TNF-α:there was no significant difference in IL-6 and TNF-αlevel between the two groups before treatment(P>0.05),and after treatment,the level of IL-6 and TNF-αin the observation group were significantly lower than that in the control group(P<0.05).3.Comparison of NIHSS score,Barthel Index and m RS score:There were no significant differences in the NIHSS score,Barthel index and m RS score between the two groups before treatment,after treatment,and at 14d,30d,and 90d after treatment(P>0.05).After treatment and 14 days after treatment,the NIHSS score,Barthel index and the difference before treatment in the observation group were significantly different from those in the control group(P<0.05),and there was no statistical difference between the observation group and the control group in the m RS score and the difference before treatment(P>0.05).There were significant differences in NIHSS score,Barthel index,m RS score and before treatment difference between the two groups after30 days treatment(P<0.05).There was no significant difference in NIHSS score,Barthel index,m RS score and before treatment difference between the observation group and the control group after90 days treatment(P>0.05).4.Comparison of the incidence of adverse reactions:There was one case(2.3%)of diarrhea in the control group,and no adverse reactions occurred in the observation group,and the difference was not statistically significant.Conclusions Hydrogen inhalation therapy can reduce the levels of serum IL-6 and TNF-αin patients with acute cerebral infarction,inhibit the inflammatory response after cerebral infarction,and improve their neurological deficit and life ability. |
