Migraine is one of clinical common chronic diseases of the nervous system.The main characteristic is unilateral or bilateral throbbing headache,at the same time may be accompanied with autonomic nervous system dysfunction symptom,such as nausea,vomiting,photophobia,fear of the sound.Resulting in a decline in the quality of life of the patients,and cause anxiety,depression and other negative emotions,as well as a variety of risk factors for cardiovascular disease,bring burden to family and society,so the research of the disease is becoming more and more important.In terms of the pathogenesis and diagnosis of the disease,fMRI has the characteristics such as noninvasive,high resolution.Widely used in the Imaging studies of all kinds of diseases in recent years.In the aspect of TCM treatment of migraine,the clinical application of Naoan pills has been used for 15 years since it was approved in 2003,the clinical treatment of migraine has certain effect.This research will be based on dyn-ICA of fMRI technology to compare the function connection difference of the brain network between migraine patients and healthy person.At the same time,exploring the brain functional imaging characteristics on the qi deficiency and blood stasis syndrome of TCM in migraine.In addition,we will observe and evaluate the effect of Naoan pills on headache attack and systemic symptoms on patients with migraine.Objective(1)Explore the mechanism of change of functional connection between brain networks in migraine.(2)Explore the imaging features of qi deficiency syndrome and bl ood stasis syndrome in migraine patients.(3)Evaluate the clinical efficacy and safety of Naoan pills in the treatment of migraine.(4)Evaluate the improvement of systemic sym ptoms on migraineurs who used Naoan pills.Methods(1)According to the discharge standard in migraine patients as the research object,take RS-fMRI examination and scale evaluation(headache diagnosis standard)on patients.selected the health control group matched patients with migraine on gender,age,and take head RS-fMRI check.Useing dyn-ICA(dynamic independent component analysis)methods to analyze the data on two groups,and explore the change mechanism of function connection between brain network,at the same time by the scale of headache diagnosis standard,trying to find brain function imaging characteristics on migraine patients with qi deficiency and blood stasis.(2)Adopt forward-looking design,take the migraine patients as research object according to the criteria.Take the physical and chemical inspection on the enrolled day.The intervention methods is Naoan pills(orally,20 pills each time,2 times a day,for four weeks),On the same day to grant pills,dose for 4 weeks.Using SPSS20.0 statistical package for statistical analysis,all of the statistical test adopt bilateral inspection,data using a paired T test or nonparametric test,counting data using X2 test,The statistically significant difference is P<0.05.The main efficacy index is attack days,frequency and duration,the secondary efficacy index based on TCM diagnostic information contains systemic symptom.Results(1)Compared with the healthy brain network,migraineurs have enhanced on f unctional connectivity as follows:SMN.1.R-SMN.2.R,SMN.2.R-DAN.2.R,DAN.1R-SN.2.L,Migraineurs have relatively weak on functional connectivity as follows:SN.1.L-D MN.4.L,SN.1.R-DMN.4.L,LN.1.R-LN.2.R,CEN.1.L-VN.1.R,DAN.1.R-VN.1.R.(2)The correlation between qi deficiency syndrome score and brain network on migraineurs was positively correlated on DMN.3.L-SN.1.R.Negative correlation includes:SN.2.R-CEN.3.The correlation between blood stasis syndrome score and brain network on migraineurs was positively correlated SMN.1.L-LN.1.R negative correlation includes:SN.2.R-DMN.3.R,SN.2.R-CEN.1.L,SN.2.R-CEN.LL,SN.2.L-VN.2.R.(3)After 4 weeks of treatment,the number of headache attacks,number of days,duration of migraineurs between pretreatment were significantly different(P<0.001).(4)After 4 weeks after treatment,there is statistical significance(P<0.05)on the sympto ms such as smell excitant odour,aggravating induced headache,stopper,after pressing alleviate pain,headache,complexion dark,cold limbs jue afternoon attack,head of the ring,drowsiness sleepiness,mood,tongue with petechiae,ecchymoses compared wi th pretreatment.Conclusions(1)There are multiple abnormal function connection of brain network between patiants and health controls,such as,the DMN,DAN,SN,SMN,CEN.(2)There is unique functional brain imaging features in qi deficiency syndrome and blood stasis,the qi deficiency syndrome with behavior and pay attention is related to CEN,SN,DMN,The blood stasis syndrome as pain treatment is related to SMN,SN,etc.(3)Naoan pills has certain curative effect on migraineurs,the specific symptoms are as follows:smell excitant odour,aggravating induced headache,stopper,after pressing alleviate pain,headache,complexion dark,cold limbs jue afternoon attack,head of the ring,drowsiness sleepiness,mood,tongue with petechiae,ecchymoses.These symptoms of treatment has a significant improvement.The symptoms are accompanied with TCM syndrome characteristics of qi deficiency and blood stasis.
