Clinical Observation Of Moxibustion Combined With Three-step Analgesia In The Treatment Of Mild To Moderate Cancer Pain

Background: pain is one of the most common accompanying symptoms in patients with malignant tumor.Pain has been listed as the fifth vital sign in modern medicine.Long term pain can cause complex pain syndrome or central pain,making common pain more severe and difficult to treat,which will seriously affect the quality of life of patients in physical,psychological,spiritual and social aspects.Although the World Health Organization(WHO) three-step pain relief principle has become an internationally recognized treatment for cancer pain,there are still some patients whose pain control is not ideal.After years of clinical research and practice,traditional Chinese medicine has been recognized in the treatment of cancer pain,Moxibustion at Zusanli,Qihai and Guanyuan is widely used in the treatment of pain in other disciplines,so we explore the role of moxibustion at Zusanli,Qihai and Guanyuan in the treatment of cancer pain.Objective: To compare the efficacy of moxibustion at Zusanli,Qihai and Guanyuan points combined with three-step analgesic method in the treatment of mild to moderate cancer pain and three-step analgesic method alone,and to evaluate the pain relief and the impact on the quality of life of the two groups,so as to provide clinical basis for moxibustion in the treatment of cancer pain.Methods: 60 patients with cancer pain were selected and assigned to moxibustion combined with three-step analgesia group(hereinafter referred to as the experimental group)30 patients and simple three-step analgesia group(hereinafter referred to as the control group)30 patients according to their own wishes.The control group was treated with mild pain(1-3): loxoprofen sodium(60mg / tablet),one tablet at a time,q12 h,7 days.In the course of treatment,some patients with mild pain can be tolerated,can reduce or temporarily do not take drugs.Moderate pain(4-6): Acetaminophen oxycodone tablets(330mg / tablet)as the representative drug,one tablet at a time,q12 h,7 days.If the pain relief is not obvious,it can be increased appropriately,no more than 1980mg(6 tablets)per day.The experimental group was treated with Moxibustion on the basis of the control group,the main points:shuangzusanli,Qihai,Guanyuan.Matching points: select the corresponding Ashi points according to the pain parts.The pain relief rate,the use of painkillers,NRS score,KPS score and quality of life(EORTC-QLQ)before and after treatment were compared between the two groups.All data were analyzed by SPSS15.0 statistical software.Result:1.After treatment,both groups were effective in the treatment of pain,and the pain relief rate of the experimental group was higher than that of the control group(P < 0.05),indicating that the analgesic effect of the experimental group was better.2.Moxibustion combined with three-step analgesic drugs can significantly reduce the dosage of analgesic drugs compared with simple three-step analgesic treatment.3.After treatment,the NRS score of the experimental group was significantly higher than that of the control group(P < 0.05),and the NRS score of the experimental group was significantly lower than that of the control group(P < 0.05),indicating that both groups can reduce the NRS score of patients,and moxibustion combined with three-step analgesia has better effect on patients with moderate cancer pain.4.After treatment,both groups can improve the KPS score of patients,and the increase value of KPS score in the experimental group is higher than that in thecontrol group(P < 0.05),indicating that the improvement of KPS score in the experimental group is higher than that in the control group.5.After treatment,moxibustion combined with three-step analgesic therapy can improve the quality of life of patients,and there are significant differences in physical function,emotional function,fatigue,nausea and vomiting,pain,insomnia and appetite between before and after treatment(P < 0.05).6.After treatment,through the comparison of the two groups of patients’ quality of life scale,it can be found that there are significant differences between the experimental group and the control group in the fields of emotional function,fatigue,nausea and vomiting,pain,insomnia and appetite(P < 0.05),indicating that moxibustion combined with three-step analgesia is better than simple three-step analgesia in improving patients’ emotional function,fatigue,nausea and vomiting,pain,insomnia and appetite Pain treatment.Conclusion:Moxibustion at Zusanli,Qihai and Guanyuan points combined with three-step analgesic method is an effective method to control cancer pain.Compared with simple three-step analgesic method,it can not only improve the analgesic efficiency,but also reduce the dosage of analgesic drugs and improve the quality of life of patients,which can be used as one of the means to control cancer pain.
