| As a common disease in pediatrics,modern doctors have accumulated rich experience in the treatment of diarrhea.However,at present,the carding and understanding of ancient literature on infantile diarrhea is not perfect,and there is less systematic research on the theoretical framework of diagnosis and treatment of infantile diarrhea,It is of great significance for the development of clinical diagnosis and treatment.This research is the content of the national "973" project sub topic "Research on the framework of TCM clinical diagnosis and treatment theory"(No.2013CB532004).In this study,using the research method of philology,I selected representative pediatric monographs from the pre Qin and Han Dynasties to the Ming and Qing Dynasties to systematically sort out,summarize the causes,mechanisms,syndromes,treatment and diagnosis theories of infantile diarrhea,and form the theoretical framework of diagnosis and treatment of infantile diarrhea.In this paper,the name of infantile diarrhea,the cause of infantile diarrhea,the pathogenesis of infantile diarrhea,the syndrome differentiation of infantile diarrhea,the treatment of infantile diarrhea,the sequelae of infantile diarrhea and the theoretical framework of diagnosis and treatment of infantile diarrhea are discussed.This paper discusses the evolution process of the name of infantile diarrhea,summarizes the exogenous and internal injury factors leading to infantile diarrhea,summarizes the pathogenesis of deficiency and excess of infantile diarrhea,discusses the syndrome differentiation and treatment methods of infantile diarrhea based on the combination of local and whole,and puts forward four sequelae of infantile diarrhea,and finally forms the theoretical framework of diagnosis and treatment of infantile diarrhea.The conclusions are as follows1.The disease names of infantile diarrhea are complex,which have experienced the evolution of naming "Xie","Li" and "Xie".The number of disease names has gradually increased,and most of them are named around the factors of etiology,pathogenesis,stool characteristics,new and long-term disease,etc.2.Children’s diarrhea is often caused by deficiency of spleen and the superabundance of liveris,weakness of spleen and stomach,external wind,cold,heat,dampness,internal injury,cold or heat,stagnant diet,panic and phlegm.Its pathogenesis mainly includes six kinds: weakness of the spleen and stomach,deficiency cold of the spleen and stomach,stagnation of diet,dampness trapping the spleen,dampness and heat of the spleen and stomach,and liver restricting the spleen.3.The syndrome differentiation of infantile diarrhea is based on the principle of deficiency and excess of cold and heat,and the combination of part and whole.In ancient times,most doctors used to differentiate the stool color and character,mainly including white stool color,yellow stool color,red stool color,black stool color,green stool color and brown stool color.White is in charge of cold and spleen deficiency.Yellow stool color can be normal stool color,also for the hot.If the heat is aggravated,the stool will turn red.If it is extremely hot,the stool will turn black.Yellow,red and black of stool color showed increasing heat potential.The color of stool is green,and it can be cold or hot.If it is hot,it will be thick,and if it is cold,it will be thin.Most of them are caused by fright and liver wood restricting spleen.The color of stool is brown,mostly due to spleen deficiency.In addition,infantile diarrhea can also distinguish cold and hot,Yin and Yang through urination,new and long-term disease,thirst or not,season and other aspects.4.The main syndromes of infantile diarrhea are spleen deficiency diarrhea,cold diarrhea,kidney diarrhea,heat diarrhea,cold and heat diarrhea,accumulated diarrhea,watery diarrhea,convulsive diarrhea,typhoid diarrhea,thirsty diarrhea and phlegm diarrhea,It includes invigorating the spleen and replenishing qi,dispersing wind and relieving exterior,warming the middle-jiao and invigorating the spleen,warming the kidney and warming the spleen,ascending Yang and stopping diarrhea,regulating the middle-jiao of Qi,promoting diuresis and dampness,removing stagnation and stasis,astringent bowel and stopping diarrhea,and calming the liver.5.Infantile diarrhea can be transformed into convulsion,dysentery,turgor and infantile malnutrition due to body fluid loss or retention of dampness.Infantile diarrhea should be treated as early as possible to prevent transmission.6.Based on the cause,mechanism,syndrome and treatment rules of infantile diarrhea in ancient literature of traditional Chinese medicine,the theoretical framework of diagnosis and treatment of infantile diarrhea is constructed around the four syndrome characteristics of deficiency,accumulation,dampness and convulsion.The deficiency of spleen and the superabundance of liveris are the physiological characteristics of children.Children’s spleen is often insufficient,so they are vulnerable to external feelings and internal injuries,leading to spleen loss of health,stagnation of water and diet,and diarrhea.Children’s liver always in superabundance,easy to panic,disturbance of liver wind,the liver restricting the spleen and become a terrified diarrhea.Deficiency,accumulation,dampness and convulsion are all closely related to the transportation and transformation function of the spleen.The treatment takes tonifying deficiency and removing excess as the treatment principle,and the purpose is to restore the physiological function of the spleen.If the Qi is cleared and the body fluid is distributed,and the turbid fluid is discharged,the stool will be naturally moistened and moderately dry without diarrhea.This study clarified the historical origin and evolution process of the development of infantile diarrhea theory,systematically combed the disease name,etiology,pathogenesis,syndrome differentiation,treatment,sequelae and other related contents of infantile diarrhea,established the framework of infantile diarrhea diagnosis and treatment theory for the first time,and provided literature support and theoretical basis for clinical syndrome differentiation and treatment of infantile diarrhea. |
