The parathyroid gland is an indispensable organ that regulates blood calcium concentration in human body.How to effectively identify this organ to avoid damage or removal of normal ones in surgery is a major problem at present.Since the discovery of autofluorescence of this organ,laser-induced fluorescence detection(LIF-D)technology is widely applied to rapid and nondestructive identification of it during surgery.To address the problems such as complex system,poor practicability and poor anti-interference ability of existing devices,this study proposed an intraoperative parathyroid identification method by using a miniature LIF-D device with optical fiber probe to bright-field detection of autofluorescence intensity.In order to solve the problems of insufficient sensitivity and low stability of traditional optical fiber-based LIF-D detectors,a 20° structure has been designed that increased the sensitivity by about 53.4% and improved the stability greatly.And a dichroic mirror components is added in the optical path to reduce the baseline to 1/30,the noise to 1/13,and increased the signal-to-noise ratio(SNR)by about 9 times.As a result,a laboratory high-sensitivity near-infrared fluorescence detector was developed.The limit of detection(LOD)reached 5.1×10-14 mol/L(Dye CF790,peak-peak noise,without enrichment).The performance such as sensitivity and stability reached the most advanced level of similar instruments.In the actual sample detection,the identification rate of parathyroid glands is83.3%,which realized the discrimination of fluorescence differences of diseased parathyroid glands.Using this instrument,researchers can test a large number of parathyroid samples and obtain accurate data to support researches on human parathyroid health.Furthermore,a miniature instrument is established.A bright-field high-sensitivity fluorescence detection technology is developed,which met the requirements of low-light detection while resisting the interference of surgical and ambient light.In clinical trials,this instrument achieved rapid,non-destructive and accurate intraoperative identification,which played a positive role in improving the success rate of surgery and proved high commercial promotion value.
