In recent years,China’s hazardous chemical industry has developed rapidly,but it has also brought about frequent accidents,which have had a significant impact on people’s lives,property,and the environment.Learning from accidents is an effective way to prevent hazardous chemical accidents.However,the current problem is that there is a lack of comprehensive accident cause analysis and efficient data collection and processing methods,which hinders the development of hazardous chemical accident cause analysis work.In response to these problems,this thesis first selects 24 Model as the theoretical basis for accident cause analysis work and uses literature review analysis,empirical research,and other methods to modify the model to make it more suitable for hazardous chemical accident cause analysis.This results in the development of a 24Model-HC accident causation model that includes six modules: unsafe actions,unsafe states,capacity-related factors,system-related factors,government regulation-related factors,and legal-related factors.NVivo qualitative analysis software is then used as an auxiliary tool for data collection and analysis during accident cause analysis.The combination of24Model-HC and NVivo forms a method for analyzing hazardous chemical accidents,which is validated using the "Tianjin Port ’8.12’ Ruihai Company Hazardous Materials Warehouse Explosion and Fire Accident" as a typical case.Finally,based on24Model-HC and NVivo,the causes of 50 hazardous chemical accidents that occurred in China between 2014 and 2021 were analyzed,and macroscopic patterns of accidents were studied from four aspects: time regularity,accident level,accident occurrence area,and accident type.Additionally,the causes of accidents were statistically analyzed based on the six modules of 24Model-HC.Research findings show that regarding macro-level patterns of accidents,July and August are the peak months for hazardous chemical accidents,and accident-prone enterprises are mainly concentrated in three major chemical provinces: Jiangsu,Zhejiang,and Shandong.Explosions are the most common type of accident,followed by explosions and poisoning incidents.In terms of statistical results for accident causes,failure to obtain a special operation permit,insufficient risk awareness,the lack of the "safety first" policy,deficiencies in the understanding of the importance of safety,inadequate regulatory oversight,and incomplete legal regulations are the most frequently occurring accident causes in the six modules of the 24Model-HC.Thus,these issues should be given priority in controlling accidents.In this thesis,an accident cause model was innovatively established for the cause analysis of hazardous chemical accidents,and the qualitative analysis software NVivo was applied to the process of accident cause analysis,which improved the comprehensivity and efficiency of accident cause analysis.The research results are beneficial for enterprises and government departments to enhance their understanding of accidents from the perspective of accident causation models.It can also help to improve organizational risk prevention and hazard investigation and control capabilities,and provide theoretical and methodological support for the effective prevention of hazardous chemical accidents,ultimately achieving precise prevention.
