Objective:Retrospective and prospective analysis infertile patients with PCOS in Dalian Municipal Women and Children’s Medical Center,to identify the best IVF-ET regimen for PCOS women,we compared clinical outcomes in different embryo-endometrial regimens among infertile women with PCOS.Subjects and Methods:Retrospective analysis of 2010-2015,Dalian Women and Children Medical Center PCOS infertility patients with fresh embryo transfer and freeze-thaw embryo transfer cycle number,pregnancy rate,delivery rate;prospective comparative analysis April 2015-August 2015,Dalian City Women and Children Medical Center treatment of PCOS infertility patients in accordance with the endometrial preparation program is divided into three groups:the natural cycle group,hormone supplement group,ovulation the cycle group.And then according to TCM syndromes will be divided into two groups:Phlegm syndrome group,non-phlegm syndrome group.Observe the baseline indicators of patients:age,infertility,BMI,sex hormone levels,ultrasound.The main outcome indicators:biochemical pregnancy rate,clinical pregnancy rate,abortion rate.Secondary outcome indicators:endometrial thickness before transplantation,endocrine levels before transplantation,preterm birth rate,low birth weight birth rate.Compare the results to statistical analysis.Results:1.The number of IVF cycles in PCOS infertility patients increased year by year from 2010 to 2015,and the number of frozen thawed embryo cycle increased more rapidly.The pregnancy rate and delivery rate of frozen thawed embryo transfer cycles were higher than those of fresh embryo transfer cycles.2.From April 2015 to August 2015,163 PCOS patients without statistical differences in baseline were treated with FET.The ratio of preterm infants between the three groups:no statistically significant(P>0.05).Comparison of clinical pregnancy rates between the three groups:ovulation the cycle group(50.6%),natural cycle group(29.0%)and hormone cycle group(33.3%)were statistically significant(P=0.045).Comparison of biochemical pregnancy rate between the three groups:ovulation the cycle group(3.7%),natural cycle group(16.1%)and hormone cycle group(13.7%)were statistically significant(P=0.046).Comparison of three groups of miscarriage rates:ovulation the cycle group(3.7%),natural cycle group(16.1%)and hormone cycle group(11.8%)were statistically significant(P=0.035).Comparison of three groups of low birth weight rate:ovulation the cycle group(20.3%),natural cycle group(66.7%)and hormone cycle group(40.0%)were statistically significant(P=0.035).3.From April 2015 to August 2015,among cycles in ovulation induction,patients with phlegm syndrome had lower pregnancy rate(33.3%vs.60.8%)and higher miscarriage rate(10.0%vs.0%),biochemical pregnancy rate(10.0%vs.0%)and preterm birth rate(10.0%vs.0%)than non-phlegm syndrome,were statistically significant(P<0.05).Conclusion:1.In recent years,infertile women with PCOS under IVF-ET in frozen thawed embryo transfer had higher selectivity,pregnancy rate and delivery rate.2.Infertile women with PCOS undergoing FET by ovulation induction cycles with embryo-endometrial regimen had a higher clinical pregnancy rate and a lower biochemical pregnancy rate and miscarriage rate.While the natural cycle of the embryo-endometrial regimen had poor clinical outcome.3.Among infertile women with phlegm syndrome PCOS undergoing FET had lower clinical pregnancy rate and a higher miscarriage rate,biochemical pregnancy rate and preterm birth rate.
