Therapeutic Effects Of Qing Fei Tong Luo Method By Song Kang On Interstitial Pneumonia With Autoimmune Features

Objective Through clinical symptom score and lung function index before and after treatment,to explore the occurrence of interstitial pneumonia with autoimmune features(IPAF)and to observe the effect of Qing Fei Tong Luo method by Song Kang on IPAF.Methods IPAF patients were collected from January 2018 to December 2018 in the respiratory outpatient department of Zhejiang Traditional Chinese Medicine Hospital.The general information,serology of autoantibodies,pulmonary function and imaging data of the patients were collected,and the clinical characteristics of IPAF were summarized.The participants were treated with the decoction of Qing Fei Tong Luo method for 3 months regularly.The modified medical research council scale(MMRC)score,traditional Chinese medicine clinical symptom score and the estimated percentage of forced vital capacity(FVC%),estimated percentage of total lung capacity(TLC%),estimated percentage of carbon monoxide diffusing capacity(DLCO%)、 the rate of carbon monoxide diffusing capacity(DLCO%/VA)before and after treatment were compared to evaluate the clinical effect.Adverse reactions during treatment were recorded.Results(1).23 patients with IPAF were included in the study,most of them were middle-aged and elderly people with an average age of 62.39±8.601.Women were more common,accounting for 60.87% of the total.The main clinical manifestations of respiratory system were cough,expectoration and shortness of breath,and the main extrapulmonary symptoms were dry mouth,dry eyes and joint pain.The most common positive autoantibodies were anti-nuclear antibodies(ANA),accounting for 60.87%,followed by rheumatoid factor(RF)and anti-sjogren’s syndrome A antigen,SSA(anti-SSA),accounting for 39.13% and21.74% respectively.The main pulmonary ventilation function of IPAF was limited ventilation dysfunction(26.09%)and moderate diffusion dysfunction(47.83%).Nonspecific interstitial pneumonia(NSIP)was the most common manifestation of lung high resolution CT(HRCT),accounting for 86.96.(2).After treatment of Qing Fei Tong Luo method,the improvement rates of Chinese medicine clinical symptom score and MMRC were 73.92% and39.13% respectively,and the symptom scores of cough,expectoration,wheezing,fatigue,dry mouth and dry eyes and MMRC scores were statistically significant(P < 0.05)while the pulmonary function indexes FVC%,TLC%,DLCO%,DLCO%/VA were not statistically significant(P > 0.05).(3).Adverse reactions: There was no adverse reaction in the course of study.Conclusion(1)The majority of patients with IPAF are middle-aged women.The common respiratory symptoms are cough,expectoration and shortness of breath while extrapulmonary symptoms were mainly dry mouth,dry eyes and joint pain.ANA,RF and anti-(SSA)were the main autoantibodies.NSIP is the most common pulmonary imaging,and the pulmonary function was restricted ventilation dysfunction and diffuse dysfunction.(2)The treatment of Qing Fei Tong Luo method proposed by professor song kang is effective in the treatment of IPAF:(1)it can maintained the pulmonary function at a certain level;(2)it can significantly improve the cough,expectoration,wheezing,fatigue,dry mouth,dry eyes and other clinical symptoms of IPAF patients,and reduce the degree of joint pain;(3)it can reduce the degree of dyspnea in IPAF patients.(3)The method of Qing Fei Tong Luo combined is safe and worth popularizing.
