Objective: Gestational diabetes mellitus(GDM)is one of the most common diseases in pregnant women,mainly due to abnormal glucose metabolism in the middle and late pregnancy.Epidemiological studies show that the incidence of GDM is increasing year by year in the world,which has adverse effects on the short and long-term health to both maternal and children.With the prolongation of the reproductive age in China,the problems of maternal age,second child,women who planned to pregnant overweight or obesity are becoming more and more serious.GDM has gradually becoming a public health problem that cannot be ignored within 1000 days of life stage.However,the nutrition management for pregnant women with GDM,China is still in a single,systematic and standardized stage,and there is no reference for the work of GDM pregnant women in the blood sugar control stage.Therefore,this study adopts a questionnaire survey combined with all-round management to provide a scientific and systematic management model for GDM women’s blood glucose control and fetal birth outcomes to ensure maternal and children’s health.Methods: According to the inclusion and discharge standards,GDM women in hospital who were voluntary participated selected to conduct comprehensive comprehensive management through questionnaires,dietary review,clinical nutrition outpatient,Multiple Disciplinary Team(MDT)guide,mobile medical care,etc.Paired research design by using self-control,using the questionnaire,Epidata 3.1 to establish the database;the application of integrated Chinese and Western medicine nutrition therapy computer expert system to calculate nutrient intake;using SPSS 20.0 for statistical description and analysis.Results: 1.80 cases of GDM women have high risk factors such as high childbearing age,childbirth history,pre-pregnancy BMI overweight or obesity,GWG too fast,etc;2.During nutritional KAP,dietary review,food frequency survey,GDM women have good attitude(93.75%),but knowledge(65%)and behavior(62.5%)needs to be improved.The diet structure is unreasonable,energy carbohydrates and protein intake are insufficient,but fat is excessive(P<0.05),there are bad eating models such as take-out,office meal,eating preferences,unreasonable cooking methods;3.Through systematic nutrition management,GDM women had improvement in KAP(P< 0.01),Fasting and postprandial blood glucose reached 56.3% after 2 weeks,and 77.5% after 4 weeks,and the incidence of cesarean section and giant child was lower..Conclusions: 1.Systematically management is recommended to pregnant women who are overweight and obese before pregnancy as soon as possible,in order to avoid the occurrence and development of GDM;2.GDM women’s attitudes and practice are still need to be improved;3.Clinical nutrition outpatient,MDT guide,mobile medical care,follow-up the integrated systemic nutrition management model can improve the KAP of GDM pregnant women and has positive significance for improving their blood glucose level.It is operable and sustainable in hospitals.
