| Objective1.To evaluate the cardiac function of patients with ejection fraction preserved heart failure(HFpEF)by echocardiographic multi-parameter score,and to characterize the multi-parameter score of HFpEF.2.To study the correlation between echocardiographic multi-parameter scores and other cardiac function indicators in HFpEF patients.3.To evaluate the curative effect of aerobic exercise therapy by changing the multi-parameter score of echocardiography in HFpEF patients.MethodHFpEF patients are originally from Shanbei Street of Wuxi who received family rehabilitation treatment or hospitalized in Rongjun Hospital of Jiangsu Province,among which 38 HFpEF patients were classified as intervention group and 33 HFpEF patients were classified as control group..The clinical data of these two groups were collected.All the subjects were given routine anti-heart failure treatment,and the intervention group was given aerobic exercise therapy for 4 months.NT proBNP,NYHA,6-minute walking test,and cardiac hyperindex scores were performed in the intervention group before and after 4 months of aerobic exercise.NT proBNP,NYHA,6-minute walking test,and cardiac hyperindex scores were also performed in the control group before and after 4 months of conventional anti heart failure treatment.1.The composition of multi-parameter scoring index of HFpEF echocardiography was analyzed.2.The correlation between multi-parameter scores of HFpEF echocardiography and NYHA classification,6-minute walking test results and NT-proBNP test results was analyzed.3.The mechanism of health benefits of aerobic exercise therapy for HFpEF patients was analyzed.4.The efficacy of aerobic therapy for HFpEF patients by multi-parameter score of HFpEF echocardiography was evaluate.Result1.Before aerobic exercise,the left ventricular remodeling index,left ventricular diastolic function and total score in the intervention group were positively correlated with NYHA scores(correlation coefficients were 0.255,0.373 and 0.289,respectively,P<0.05).The total score of HFPEF was negatively correlated with the 6-minute walking test distance(correlation coefficient was-0.255,P<0.05)The results of NT proBNP showed a positive correlation with the total score of HFPEF(0.293,0.332,0.257,0.279,0.321,P<0.05).2.After aerobic exercise,the total score of HFPEF and NYHA in the intervention group showed a positive correlation(correlation coefficient 0.283,P<0.05),the total score of HFPEF and 6-minute walking distance showed a negative correlation(correlation coefficient-0.268,P<0.05),the left atrial enlargement,left ventricular diastolic function and right ventricular function index of HFPEF There was a positive correlation between NT proBNP and standard,PSA and total score(correlation coefficient was 0.355,P<0.05).3.After aerobic exercise treatment,the levels of BMI,systolic blood pressure,diastolic blood pressure,blood sugar and triglyceride in the intervention group decreased significantly(P<0.01);the heart function of the intervention group was improved significantly,the levels of NYHA grade and NT-proBNP decreased significantly,and the level of 6-minute walking test increased significantly.4.After aerobic exercise treatment,echocardiographic cardiac structure and function were improved in the intervention group.There were significant differences in IVST,LAD,E,A,E/A,E/Ea and PAS(P<0.01),and there were also significant differences in Ea(P<0.05).Echocardiographic multi-parameter scores were improved,mainly in the indexes of left ventricular remodeling(P<0.05),left ventricular diastolic function,PAS and multi-parameter scores(P<0.01).Conclusion1.HFpEF echocardiographic multi-parameter score can accurately and objectively reflect the changes of cardiac structure and function.It has significant correlation with the main indicators of cardiac function,including NYHA grading,6-minute walking test level and NT-proBNP level.2.For HFpEF patients,aerobic exercise can improve exercise endurance and cardiac function.It can also reduce weight,blood pressure,blood sugar and lipid,reduce and prevent some risk factors of cardiovascular diseases.3.The multi-parameter score of HFpEF echocardiography can be used to evaluate the effect of aerobic exercise on HFpEF patients.It is found that aerobic exercise mainly improves left ventricular diastolic function,relieves left ventricular remodeling,reduces pulmonary artery pressure and improves right ventricular function in HFpEF patients.Besides,the structure and function of heart can be improved. |
