Objective: To analyze the relationship between the RE of GD-EOB-DTPAenhanced MRI hepatobiliary stage of hepatocellular carcinoma on a chronic liver background and the differentiation of hepatocellular carcinoma,the uptake of transporter OATP1B3,and the release of transporter MRP2 and MRP3.Materials and methods: The subjects of the study were 76 patients with chronic liver background who completed Gd-EOB-DTPA enhanced MRI examination and were pathologically confirmed as liver cancer in the First Affiliated Hospital of Guangxi Medical University.including 64 males,12 females,males and females The ratio was 64:12,they are between 16-78 years old,with an average age of 52.68±12.43 years old.All patients included in the study underwent liver Gd-EOB-DTPA enhanced MRI and liver plain scan within one month before surgery.Calculate the degree of SI enhancement in the hepatobiliary phase,the liver-muscle ratio(LMR)of the liver and erector spinae,the calculation formula is: scan LMRpre=SItum pre/SImpre before enhancement,scan LMRpost=SItumpost/SImpost after enhancement,relative enhancement of liver Degree(relative liver enhance,RLE)RLEm=LMRpost-LMRpre,and relative liver enhancement rate(enhancement ratio RE)RE=LMRpostLMRpre/LMRpre.According to the signal level of the hepatobiliary phase,the lesions are divided into two types,namely low signal HCC(SItum/SIliv<1)and equal or high signal HCC(SItum/SIliv>=1).Collect 76 patients’ liver cancer tissue specimens and corresponding adjacent normal tissue specimens for HE staining to analyze the correlation between differentiation and hepatobiliary stage enhancement rate.RT-PCR and immunohistochemical experiments were performed on liver cancer tissue specimens from 76 patients and corresponding normal tissue specimens adjacent to cancer to detect the expression of uptake transporter OATP1B3 and excretion transporter MRP2 and MRP3.Analyze the correlation between differently differentiated hepatocellular carcinoma and the enhancement rate of hepatobiliary stage,and analyze the correlation between the expression of uptake transporter OATP1B3 and excretion transporter MRP2,MRP3 and the enhancement rate of hepatobiliary stage.Results:(1)The average enhancement rate of 76 cases of hepatocellular carcinoma tissue in the hepatobiliary stage was 0.66±0.77.The average RE of 12 cases of high signal group liver cancer in the hepatobiliary stage was 2.06±0.71,64 cases of low signal group liver cancer in the hepatobiliary stage was 0.39±0.41(P<0.01).The RE of the high signal group was significantly higher than that of the low signal group,and the difference was significant(P<0.05).There is no significant correlation between the differentiation of hepatocellular carcinoma and the HP RE of Gd-EOB-DTPA-enhanced MRI(P>0.05).(2)The RT-PCR results of the uptake transporter OATP1B3 showed that the expression in the high signal group was 1.91±0.94,and the expression in the low signal group was0.13±0.19(P<0.01).The results of immunohistochemical experiments showed that in the 12 cases of hyperintensity group,10 cases of the transporter OATP1B3 expression level were 3,1 case was the expression level 2,and 1 case was the expression level 1.In the 64 cases of hypointensity group,there was 1 case of OATP1B3 with an expression level of 3,4 cases with an expression level of 2,21 cases with an expression level of 1,and 38 cases with an expression level of 0.The expression of transporter OATP1B3 relative to the adjacent tissues was significantly related to the signal intensity of the hepatobiliary stage.As the expression of OATP1B3 increased,the enhancement rate of the hepatobiliary stage also increased(R=0.79,P <0.01).(3)RT-PCR of the transporter MRP2 showed that the expression of the high signal group was 0.82±0.37,and the expression of the low signal group was 0.93±0.79(P>0.05).In the high signal group,5 cases of the transporter MRP2 expression level were 0,6 cases had the expression level of 1,and 1 case had the expression level of 2;in the 64 cases of low signal group,there were 3 cases of the transporter MRP2 expression level.The expression level was 3,4 cases had an expression level of 2,17 cases had an expression level of 1,and 40 cases had an expression level of 0.No matter in the high signal group or the low signal group,the expression of the transporter MRP2 in cancer was lower,and there was no significant difference between the high signal group and the low signal group(P>0.05).(4)The expression of transporter MRP3 in the high signal group was 1.55±0.23,and the expression in the low signal group was 0.90±0.67(P<0.05).Among the 12 cases in the hyperintensity group,8 cases had the transporter MRP3 expression level 3,1 case had the expression level 2,and 3 cases had the expression level 0;in the low signal group,36 cases had the transporter MRP3 expression level.The expression level of 2cases is 3,the expression level of 2 cases is 2,and the expression level of 26 cases is 0.The expression of the transporter MRP3 relative to the adjacent tissues was positively correlated with the signal intensity of the hepatobiliary stage.As the expression of the transporter MRP3 increased,the enhancement rate of the hepatobiliary stage also increased(R=0.38,P<0.05).Conclusion:(1)There is no significant correlation between the differentiation of hepatocellular carcinoma and the RE of hepatobiliary phase of Gd-EOB-DTPA-enhanced MRI.(2)Gd-EOB-DTPA enhanced MRI hepatobiliary phase RE is significantly related to the transporter OATP1B3.With the increase of the transporter OATP1B3 expression,the hepatobiliary phase signal and enhancement rate also increase.(3)The RE of Gd-EOB-DTPA enhanced MRI in the hepatobiliary phase is positively correlated with the transporter MRP3.As the expression of the transporter MRP3 increases,the SI and RE in the hepatobiliary phase also increase.(4)Gd-EOB-DTPA enhanced MRI hepatobiliary signal intensity has no obvious correlation with the transporter MRP2.In most hepatocellular carcinomas,the transporter MRP2 expression is reduced.
