| Objective: To explore the incidence,severity and type of ulcerative colitis symptoms,analyze the influencing factors of the symptom cluster,provide objective,systematic,comprehensive and efficient reference indicators for the evaluation of ulcerative colitis symptoms cluster,and provide the basis for the formulation of elaborate intervention measures.Methods: Ulative colitis patients in a two Grade A general hospital in Nanning from July 2020 to January 2021 using convenient sampling method.The data was collected by questionnaires,and general demographic data and disease information related data sheets,classification quantification table of chronic non-specific ulcerative colitis symptoms,hospital anxiety and depression scale,and Anderson Symptoms Assessment Scale(Part II)were selected as research tools.Exploring factor analysis is used to determine the common symptom cluster of patients with ulcerative colitis,and confidence analysis is used to test the consistency of symptoms in the symptom cluster;taking the symptom cluster severity score as the factor variable,and the general demographic data and disease information as independent variables.After normal test,symptom cluster severity score does not obey the normal distribution(P<0.05),so Mann-Whitney U rank sum test,Kruskal-Wallis H rank sum test,double rank sum test,Spearman rank correlation analysis,and multiple progressive regression model were used to analyze the influencing factors of symptom cluster in patients with ulcerative colitis.Results:1.A total of 130 questionnaires were distributed,Recovery of 120 valid questionnaires.And the incidence of symptoms from high to low,it was followed by diarrhea(71.7%),burning anus(59.2%),upset fever/divergence(55.8%),abdominal pain(53.3%),abdominal distention(53.3%),acute and severe(50.8%),purulent stool(48.3%),low food intake(47.5%),burnout(46.7%),cold limbs(45.0%),dizziness(45.0%),weakness of knees(41.7%),lazy speech(37.5%),lumbar acid(28.3%),fever(15.0%),Durine(11.7%),belching(10.0%),stolen sweat(10.0%)chest swelling pain(7.5%),stool hardening(6.7%),abnormal menstruation(6.7%),skin armor(5.0%),abdominal lump(4.2%);A total score of(75.36±29.03)was obtained.A total of(381.33±184.58),The scores of symptom disturbance from high to low were:emotion,work,life fun,relationship with others,general activity and walking.2.Two symptom clusters were finally determined from the exploratory factor analysis results.According to the characteristics of the symptoms,they were named the intestinal function-related symptoms cluster and the negative state symptoms cluster,respectively.Among them,the symptoms related to intestinal function include diarrhea,pus and blood stool,abdominal pain,abdominal distension,internal anxiety,anal burning and other 6 symptoms,negative state symptoms include less food,waist acid,knee weakness,limb fatigue,fatigue,laziness,cold limbs cold,irritability,dizziness and other 8symptoms.3.The results of single factor analysis of symptom cluster showed that the severity and disease classification are different,with severity score difference of statistical significance(P<0.05),the severity of disease and medical expenses,and the difference in severity score of negative state symptom cluster of statistical significance(P<0.05).The comparison results showed that the heavier the severity of the disease,the greater the burden of UC symptoms,the least impact of the rectal cluster on intestinal symptoms in disease classification,and the least on negative symptoms;the remaining factors,such as different gender,residence,culture,disease course,disease activity,disease type,have no statistical significance(P>0.05).4.Analysis results related to the symptom cluster:Symptoms clusters positively associated with intestinal function are anxiety(r=0.379,P<0.001),depression(r=0.449,P<0.001),and negative association with HB(r=-0.181,P=0.048);Positive associated with negative state symptom cluster are anxiety(r=0.296,P=0.001),depression(r=0.262,P=0.004),age(r=0.193,P=0.035),HB(r=-0.439,P<0.001);BMI,course has no correlation with UC symptom cluster(P>0.05).5.Multi-factor analysis results of the symptom cluster: The influencing factors of the symptom cluster related to intestinal function are the severity of the disease,disease classification,anxiety and depression state(P<0.05),among which the influence of anxiety and depression state is the most significant,followed by the disease classification,and the smallest impact is the severity of the disease.Negative state symptoms clusters affect the severity of the disease,HB value,medical cost payment method,anxiety and depression status(P <0.05),in which the medical cost payment method is the most significant impact,followed by the severity of the disease,and the least impact is the HB value.Conclusion:1.Ulcerative colitis due to the characteristics of the disease,patients have a variety of symptoms,including extraintestinal,intestinal symptoms,these symptoms do not occur independently,but in a variety of symptoms coexist,therefore,medical staff should increase the symptom evaluation of patients with ulcerative colitis,improve the symptom evaluation system.2.Through the questionnaire adjustment of patients with ulcerative colitis and the exploratory factor analysis,two symptom clusters were extracted,namely the intestinal function-related symptoms cluster and the negative state symptoms cluster,which proved that the symptoms experienced by ulcerative colitis patients can constitute the symptom cluster,providing a scientific basis for the further formulation of corresponding intervention measures.3.The analysis of general demographic data,disease information,psychological factors determine the severity of the disease,anxiety depression,disease classification,negative symptoms,anxiety severity,medical payment,and hemoglobin value.Therefore,medical staff should strengthen the identification of ulcerative colitis,provide reference for a comprehensive model of efficient and accurate intervention in symptom cluster. |
