Objective: To investigate the test results of urodynamics in patients with different levels of uric acid by reviewing the clinical data and urodynamic parameters of patients with different levels of uric acid in Benign prostatic hyperplasia Benign Prostatic Hyperplasia(BPH),to provide reference for clinical work to improve the level of diagnosis and treatment of these diseases. Methods: A total of 96 cases of middle-aged and elderly male patients with Benign Prostatic Hyperplasia(BPH)were included in this study.Clinical history was collected.the age of all patients was analyzed, the results of ultrasound examination of prostate and Blood biochemical indexes such as Uric acid(UA),Fasting Blood Glucose(FPG),liver function and renal function were collected.According to the collected clinical data: 1.The patients were divided into HUA Group(N = 24)and non-HUA Group(N = 72)according to Hyperuricemia, and the differences of urodynamic parameters between the two groups were compared;2.Dividing all subjects into 4 groups based on the quartile of serum uric acid level: U1,U2,U3,U4.The uric acid values of each group were <312.0,312.0 ~ 351.0,352.0 ~ 420.0 and > 420.0μmol /L,respectively,the differences of urodynamic parameters in each group were compared.Results:1.General Situation: Among the 96 patients included in this study,43 cases(44.8%)were found to have enlarged prostate gland entering bladder, the results of urodynamic examination showed that 49 cases(51.0%)had sensitive bladder function,27 cases(28.1%)had decreased bladder compliance,52 cases(54.2%)had decreased bladder stability, and 26 cases(27.1%)had decreased bladder contractility,twenty-four patients(25.0%)were diagnosed as Hyperuricemia.2.Comparison of urodynamic parameters between non-HUA and HUA patients:(1)Compared with HUA group, the incidence of bladder sensitivity and bladder compliance decreased significantly in non-HUA group, while the incidence of enlarged prostate entering bladder decreased significantly;(2)Compared with non-HUA group, the volume of First Sensation of Urination(FSU)and Residual volume(RV)in HUA group were significantly increased, while the Maximum flow rate(Qmax)was significantly decreased;(3)There was no significant difference in the incidence of age, decreased bladder stability and decreased bladder contractility between HUA and non-HUA groups.3.The comparison of the quartile urodynamic parameters and clinical data of the high and low level of serum uric acid showed that there was no significant difference in the age of the patients in the 4 groups, but the difference of FSU,Qmax and RV was statistically significant(p<0.05),in other words, with the increase of uric acid level,FSU and RV increased gradually, while Qmax decreased gradually.Conclusions:1.Bladder hypoesthesia occurs in patients with Benign prostatic hyperplasia who have elevated blood uric acid levels or in patients with Benign prostatic hyperplasia who have Hyperuricemia.2.Bladder compliance is higher in patients with benign prostatic hyperplasia with Hyperuricemia.3.In patients with Benign prostatic hyperplasia and Hyperuricemia, enlarged prostate glands are more likely to protrude into the bladder.4.High levels of uric acid significantly affect urodynamic parameters in patients with Benign prostatic hyperplasia(BPH),worsen lower urinary tract symptoms, and speed up the progression of BPH,hyperuricemia could be a risk factor for Benign prostatic hyperplasia. |
