Objective: To compare and analyze the advantages and disadvantages of applying Hook-wire positioning needle and medical glue,and to provide a reference for selecting appropriate positioning methods for preoperative positioning of small pulmonary nodules.Methods: A retrospective analysis of 74 patients with small pulmonary nodules admitted to the Department of Thoracic Surgery of the First Affiliated Hospital of Guangxi Medical University from January 2020 to January 2021 were performed before surgery.According to the positioning method,they were divided into 51 cases in the Hook-wire group and 23 cases in the medical glue group.The positioning success rate,positioning complications(pneumothorax,hemothorax,pleural reaction,chest pain),operation time and operation success rate of the two groups were compared and analyzed.Results: Statistics of age,gender,smoking history,past tumor history,nodule size,nodule location,distance between nodule and pleura,postoperative pathological results,number of nodules in a single location,and methods of thoracoscopic surgery in the two groups of cases There was no significant difference in learning(P>0.05).The positioning was successful in 58 cases(100%)in the Hook-wire group and 26 cases(100%)in the medical glue group.There was no statistically significant difference in the rate of pneumothorax after positioning between the Hook-wire group and the medical glue group(16% vs4%,P>0.05);the rate of pneumothorax after positioning in the Hook-wire group was higher than that of the medical glue group(16% vs 4 %),there was a significant statistical difference(P>0.05);there was no statistically significant difference in pleural response(6% vs 0%,P>0.05);the chest pain score of the Hook-wire group was higher than that of the medical glue group(5.20±1.40)>3.13±1.06),there is a significant statistical difference(P<0.05).The VATS time of the Hook-wire group(121.63±65.76min)was shorter than that of the medical glue group(138.26±73.27min),and there was a statistically significant difference(P<0.05).There were 47 successful VATS cases in the Hook-wire group and 23 cases in the medical glue group.There was a statistically significant difference(84% vs 100%,P<0.05).Conclusion: Medical glue and Hook-wire preoperative positioning are both safe and effective techniques.It is necessary to choose an appropriate positioning method according to the patient’s specific situation.
