Outcomes Of Patients Initiated Peritoneal Dialysis Within 7 Days

Objective: The guidelines for peritoneal dialysis recommended that peritoneal dialysis should be initiated 2 weeks after peritoneal catheterization.However,many patients need peritoneal dialysis as soon as possible due to urgent complications.The optimumtime of starting peritoneal dialysis is controversial.This study observed the safety and prognosis of early initiation of peritoneal dialysis after catheterization.Methods:This study retrospectively analyzed the complications and catheter survival of patients who underwent peritoneal dialysis early after peritoneal catheterization in our center from January 1,2008,to December 31,2016.The average follow-up time was 3 years.According to the time of startedperitoneal dialysis after catheterization,the patients were divided into three groups: P1(≤ 7 days),P2(8-13 days),and P3(≥ 14 days).All patients used the same starting dose of peritoneal dialysis.The primary outcome of the study was catheter technical survival,the secondary outcomes included PD-related complications and death.Results:Among 549 patients,437 met the inclusion criteria,including163 cases in P1 group,189 cases in P2 group,and 85 cases in P3 group.The 1and 3-year technical survival rates in P1,P2 and P3 groups were 95.7%,92.6%and 87.1%(P=0.188);94.5%,89.9% and 85.9%,respectively(p=0.066).The 1and 3-year patient survival were 90.2%、94.2%、97.6%(P=0.188)and 81.0%、82.5%、85.9%,respectively(p=0.595).There were no significant differences in perioperative complications,infection complications,and catheter displacement ratesamong the three groups.Conclusion:Patients initiated peritoneal dialysis within 7 days were similar to patients initialed peritoneal dialysis in 8-13 days and ≥ 14 days in technique survival patient survival,and peritoneal dialysis related complications.Therefore,we suggest that starting peritoneal dialysis within 7days may be a conventional therapy times.
