Background China is facing an increasingly severe situation of chronic diseases in the older adults.The prevalence of hypertension is increasing year by year in the older adults,long-term hypertension pose a great threat to the quality of life of the patients.Previous studies have shown that the quality of life of patients with hypertension is significantly lower than those with normal blood pressure,and low quality of life may bring heavy burden to families and society.Socioeconomic status,social capital and quality of life have been proved to be positively correlated.According to the health ecology model,the higher the socio-economic status,the wider the scope of social networks,and the more convenient it is to obtain social resources,the more opportunities to improve the quality of life.We speculate that social capital may play a mediating role in the relationship between socio-economic status and quality of life.At present,few studies focus on rural elderly patients with hypertension to explore the relationship between the three.The measurement of socio-economic status rarely takes into account both objective and subjective aspects,and the role of social capital in it needs to be discussed.Objectives To investigate the quality of life of elderly patients with hypertension in rural areas of Shandong Province,the relationship between objective and subjective socio-economic status and quality of life,and to examine the mediating effect of social capital on the relationship between objective and subjective socio-economic status and quality of life.Methods The data came from "the path research on the impact of social capital on the quality of life of rural elderly patients with hypertension" carried out in 2021.A total of 950 rural older adults with hypertension more than 60 years were recruited from 3 cities(Heze Shan county,Jinan Laiwu and Weihai Rushan)in Shandong using multistage stratified cluster sampling method.The questionnaires were included a questionnaire on basic information of socio demography(including three indicators for evaluating objective socio-economic status and one item for subjective socio-economic status),Elderly hypertensive patients PRO scale(Patient Reported Outcome,)and social capital scale.Epidata 3.1 software was used for data input,and Stata 15.0 software was used for data processing and preliminary analysis.Nonparametric tests were used for univariate analysis.Spearman correlation analysis was used to analyze the relationship between objective and subjective socio-economic status and quality of life,and social capital.The mediating effect of social capital was examined using SPSS plugin,Process 2.16.Results This study found that the quality of life of elderly patients with hypertension in rural areas of Shandong Province still has great room for improvement,and the score of quality of life was 132.57± 19.40.There was a significant positive correlation between objective and subjective socio-economic status and quality of life(r=0.330 and r=0.324,P<0.001);and a significant positive correlation between objective and subjective socio-economic status and social capital,(r=0.181 and r=0.170,P<0.001);and social capital had a significant positive correlation with quality of life(r=0.196,P<0.001).On the basis of controlling the potential confounding factors,the results of mediating effect analysis showed that compared with the objective socio-economic status,the quality of life was more sensitive to the overall positive impact from the subjective socio-economic status(β=1.894 and β=6.684,P<0.001);The intermediary effect between social capital and social capital was significant,and mediating effect accounted for 7.76%and 5.67%of the total effect for objective and subjective socio-economic status respectively.Conclusion and Suggestion The overall quality of life of elderly patients with hypertension in rural areas of Shandong Province still has much room for improvement;those with higher objective and subjective socio-economic status scored higher in quality of life,,and improving the subjective socio-economic status of elderly patients with hypertension was more efficient to improve their quality of life.The higher objective socio-economic status or subjective socio-economic status of elderly patients with hypertension in rural areas of Shandong Province may improve the quality of life through a high level of social capital.Therefore,Pay attention to the income level,education and employment opportunities of elderly patients with hypertension in rural areas,;ensure that family intergenerational support and medical security are in place;Improve the ability of community elderly care service,and focus on elderly care model for rural elderly patients with hypertension. |
