Objectives:In this study,Bayesian network technology was used to extract the elements of common syndromes and TCM syndromes of severe pneumonia,to explore the TCM syndromes of severe pneumonia and the relationship between syndrome distribution and related influencing factors,so as to provide clinical basis for the standardization of TCM syndromes of severe pneumonia.Methods:In this study,a retrospective study was conducted to collect 247 patients with severe pneumonia who met the inclusion and exclusion criteria in the department of critical care medicine,wangjing hospital,China academy of Chinese medical sciences from January 2014 to December 2021,and establish the corresponding information database.EXCEL2010 was used to preprocess the collected clinical data.In the pretreatment stage,TCM symptoms and related times of patients were counted by this software.In the research stage,GeNIe2.0 Bayesian network simulation software was used for statistical analysis of the pretreatment data.Specifically,GeNIe2.0 software was used to load the data preprocessed by Excel to generate the Bayes network model diagram between symptoms with causal relationship and extract TCM syndrome elements with both subjective and objective factors.In order to determine the relationship between the syndrome elements,establish the Bayes network model among the syndrome elements,and combine expert experience to explore the types and distribution rules of the syndrome elements of severe pneumonia,determine the common TCM syndrome types of severe pneumonia,and build the Bayes network model of each syndrome type.In addition,To investigate the influence of individual,environmental and social factors on the occurrence of the disease and the distribution of TCM syndromes.Results:1.Using Genie2.0 bayesian network software for the entry of severe pneumonia symptoms of traditional Chinese medicine data structure learning,then investigates indicators of TCM symptoms and correlation dimension,and generate a causal relationship between the symptoms of the Bayes network model diagram,to show the connections between symptoms strength,including fever,cough,sputum sticky,yellow,dry mouth,constipation,Symptoms such as fatigue are closely related to other symptoms and are the main symptoms of severe pneumonia.On this basis,combined with the guidance of the expert group of the project and relevant literature,three disease location factors of lung,kidney and spleen and six disease factors of phlegm-heat,phlegm-dampness,wind-heat,Yang deficiency,qi deficiency and Yin deficiency were extracted.In the early stage of TCM syndrome differentiation of severe pneumonia,wind-heat and phlegm-dampness are the main symptoms,and the disease location is mainly in the lung,which is gradually from the lung and spleen.In the late stage,phlegm-turbidities and natural qi deficiency are the main symptoms,involving the kidney.The disease is relatively mild,visible cough,expectoration,phlegm in the throat,the symptoms of eating less and other diseases in the lungs and spleen,severe cases see shortness of breath,delirium,wheezing and other crises.By building the Bayes network model between the syndrome elements,according to disease associated with disease factor,finally extract phlegm accumulate lung syndrome,phlegm hot accumulate lungs and qi and Yin deficiency syndrome,phlegm wet lung syndrome,phlegm wet resistance lung and lung spleen deficiency syndrome,hot wind make lung syndrome,pathogenic stuck is to take off the card six TCM syndrome types,well summarized the TCM syndrome characteristics of severe pneumonia.2.The incidence of severe pneumonia was closely related to age,gender,smoking and drinking,onset time and place of residence.The patients with severe pneumonia were mainly over 60 years old,including 159 males and 88 females,with the majority of males and most of them had smoking and drinking history.The onset season is mainly in autumn and winter,when the temperature plummets and the ability of the body to resist external pathogens is reduced,which is easy to lead to the occurrence of diseases.Analysis of the patient’s basic diseases found that COPD patients 77 cases,accounted for the largest proportion,followed by diabetes,hypertension,coronary heart disease,cerebral infarction,chronic bronchitis.In addition,patients with multiple underlying diseases may suffer from prolonged and difficult recovery of severe pneumonia and increase the risk of death due to long-term use of antibiotics,or long-term bed-rest and mobility difficulties.3.By analyzing the distribution of TCM syndromes and related influencing factors of severe pneumonia,it was found that phlegm-heat accumulation of lung and deficiency of qi and Yin accounted for the largest proportion of all syndromes.In different age groups,patients under 60 years old were mainly characterized by phlegm-heat accumulation of lung,while patients over 80 years old were mainly characterized by phlegm-dampness blocking lung and syndrome of deficiency of lung qi.Male patients had more Yang and less Yin,and more smoking and drinking history.Compared with female patients,the syndrome type was more common with phlegm-heat accumulation of lung syndrome and other empirical and heat syndromes.In the seasonal distribution of the disease,the proportion of phlegm-dampness blocking lung syndrome and the syndrome shedding of evil depression in autumn and winter is higher than that in spring and summer,which is related to the cold weather and the weakening of Yang qi.Analysis of mechanical ventilation in severe pneumonia patients showed that invasive ventilation accounted for more than non-invasive ventilation,but the probability of withdrawal was lower than that of non-invasive ventilation.In terms of syndrome types,wind-heat invasion of lung and phlegm-dampness obstruction of lung syndrome were more likely to be removed,while the probability of withdrawal was lower and the prognosis was poor.Conclusions:1.In this study,Bayes network was used to explore the TCM syndrome rule of severe pneumonia,from which three disease locations of lung,kidney and spleen and six disease elements of phlegm-heat,phlegm-dampness,wind-heat,Yang deficiency,Qi deficiency and Yin deficiency were extracted.Finally,six TCM syndromes including phlegm-heat accumulation of lung,phlegm-heat accumulation of lung combined with deficiency of qi and Yin,phlegm-dampness preventing lung,phlegm-dampness preventing lung combined with deficiency of qi and qi,wind-heat invading lung,and evil depression releasing syndrome were extracted to better summarize the TCM syndromes of severe pneumonia.2.The incidence of severe pneumonia is closely related to age,smoking,gender,onset season and other factors.The incidence of severe pneumonia is higher in male patients,those with smoking and drinking history,autumn and winter seasons,and those with multiple underlying diseases.Lung and phlegm heat yun qi and Yin deficiency syndrome in severe pneumonia for TCM syndrome types each accounted for the largest,young and middle-aged patients with history of smoking,drinking,men as well as the spring and summer,more than in aggregates lung phlegm heat syndrome such as empirical and heat syndrome,and older patients,female patients and the onset of qiu dong season,many with a lung and phlegm wet resistance lung and spleen deficiency syndrome and deficiency syndrome,cold syndrome.3.Bayes network technology can simulate the clinical thinking process of physicians,mine the hidden variables of data,show the correlation degree between data,and finally extract TCM syndrome types,which has great advantages in the study of TCM syndrome laws. |
