Medical and health service that matters national economy and the people’s livelihood is now a hit topic jostling for public attention.At the moment,China is exposed to intensifying aging of population and increasing hospitalization costs.Difficulty and high expense in seeking for medical treatment demand prompt solution."Health China 2030" Planning Outline stresses the need to perfect the multi-level medical security system mainly based on basic medical security,and establish a basic medical and health system with Chinese characteristics.Basic medical insurance system is playing an increasingly significant role.In that case,studying the impact of the basic medical insurance system on relieving the economic burden arising from medical expenditures is of practical demands and great significance,while shining on boosting the high-quality progress of basic medical insurance work,making the best of public medical and health resources to serve the fast-growing demands for medical advice,and stepping up health for all.Foremost,efforts were invested to sort out respective researches by domestic and foreign scholars on disease economic burden and its influencing factors,as well as effect of medical insurance on disease economic burden.This paper is written from the perspective of whether basic medical insurance system actually works on easing the disease economic burden and relieving catastrophic medical treatment costs.Second,it expounded development of China’s basic medical insurance system,and status quo of medical and health for residents.Furthermore,theoretical basis was interpreted referring to Grossman theory of health needs and risk management theory.Then definition and scope on the disease economic burden index and basic medical insurance system were listed.This paper dissected efficacy of different medical insurance systems on the economic burden of disease in according with CHARLS data,applying the two-part model,OLS model and quantile model.In the meantime,catastrophic medical expenditures were included to expand the study,thus to figure out the impact of the basic medical insurance system on the economic burden of disease in different regions and groups with varying incomes,resulting in a prudent test.Outcomes revealed that:(1)Basic medical insurance system produces a marked effect lightening economic burden of disease(regression coefficients of employee medical insurance,urban and rural medical insurance,and new rural cooperative medical system(NCMS)were-0.253,-0.152,and-0.161 respectively,on the overall regression results;in terms of the quantile regression results,basic medical insurance of all types reported a lowered economic burden of the respondents at each quantile of the economic burden of disease after the 10%quantile.Meanwhile,heavier economic burden was accompanied by greater effect).NCMS and urban and rural residents medical insurance recorded a relatively effect in saving catastrophic medical expenses(regression coefficients of NCMS and urban and rural medical insurance were-0.327,-0.324,respectively),underlining the role in prevention and control of a serious illness.These basic medical insurance worked better in narrowing down catastrophic medical expenditure(regression coefficients of employee medical insurance,urban and rural medical insurance,resident medical insurance and NCMS were-0.311,-0.324,-0.231,and-0.327 respectively)than easing the economic burden of disease(in terms of quantile regression,impact of all basic medical insurances on catastrophic medical expenditure after the 80%quantile also worked alike).(2)There are differences in the impact of different regions on the economic burden of disease.Compared with the eastern region,the west central region has a positive impact on the economic burden of disease,especially the western part(Heterogeneity analysis revealed that regression coefficients in the western and central regions were 0.328 and 0.182,respectively in comparison with the eastern region).(3)Basic medical insurance system(Regression coefficients of employee medical insurance,urban and rural medical insurance,resident medical insurance and NCMS were-0.270,-0.241,-0.272 and-0.136,respectively)significantly lessened the economic burden of disease suffered by people in rural areas and low-income groups(employee medical insurance,urban and rural medical insurance,resident medical insurance and NCMS were-0.219,-0.237,-0.197 and-0.155 respectively).(4)Commercial medical insurance made up for basic medical insurance,yielding a remarkable effect in both maximizing the utilization of residents’ medical and health services(regression coefficient was 0.568)and alleviating the economic burden of disease(regression coefficient was-0.352).(5)Family size was found to be effective in alleviating catastrophic medical expenditure(regression coefficient was-0.180 on the overall regression results).Therefore,financial inputs should be doubled to aid the subsequent development of the basic medical insurance system,shore up the safeguard role of basic medical insurance,reduce urban-rural and regional differences;progress should be made in speeding up development of commercial medical insurance,enlivening the vitality of commercial medical insurance with market mechanism,and coordinating its relationship with the basic medical insurance;last but not least,it has to boost the reform of the basic medical insurance system,and output a basic medical security system with Chinese characteristics.
