BACKGROUNDIn recent years,the number of diabetic patients worldwide has been increasing,and the occurrence and progress of diabetes are closely related to changes in pancreatic islet βcells function.Therefore,the classification of islet function is very important for the management of diabetic patients.At the moment,there is no recognized grading of islet function for diabetic treatment all over the world.This study aims to find an optimal islet function index to grade the islet β-cell function of diabetic patients,aiming to provide a reference for the clinical treatment of diabetes patients with hypoglycemic drugs,so as to achieve the clinical treatment outcome of blood glucose standards.METHODIt is a retrospective analysis,with a total of 2,493 diabetic inpatients in the Endocrinology and Metabolic Diseases Department of Cheeloo Hospital,Shandong University,from January 2017 to March 2021.We used the cut-off points of postprandial C-peptide maximum to glucose maximum ratio of different treatment groups to grade the islet function and perform corresponding verifications,which provides a basis for the selection of diabetes treatment options.RESULTSIn the first part,the cross-sectional analysis of 567 subjects proved that PCGRmax was significantly correlated with other pancreatic β-cell function indexes and blood glucose index.In the second part,by analyzing the clinical data of 1702 subjects,proved that PCGRmax is in different treatment groups(including lifestyle modification group,nonsecretagogue treatment group,secretagogue treatment group,basal insulin treatment group and intensive insulin treatment The difference between the groups)is significant,and find the PCGRmax cut point to distinguish the different treatment groups.In the third part,through the clinical verification of the pre-discharge blood glucose standard rate of 699 subjects,proved the effectiveness of the obtained PCGRmax cut point.In the fourth part,it is proved that PCGRmax is a protective factor for different chronic complications of diabetes through the analysis of the complication data of 449 subjects.CONCLUSIONPCGRmax can be used as an indicator of islet function classification.Choosing a hypoglycemic treatment plan that matches the patient’s PCGRmax index can improve the rate of blood glucose compliance.PCGRmax is a protective factor for different chronic complications of diabetes.
