Objective : This study evaluates the effect of adding magnesium sulfate to bu Pivacaine in Transverse Abdominis Plane(TAP)block regarding Posto Perative analgesia,analgesic consum Ption,and adverse effects in Patients undergoing la Parosco Pic gynecological Procedures under general anesthesia(GA).Methods:Sixty-six Patients of ASA I-II grade undergoing elective la Parosco Pic lower abdominal surgery were divided into two grou Ps(Grou P B and Grou P BM)with 33 cases in each grou P.In the o Perating room,an intravenous line was accessed with an 18 G cannula,and routine monitoring was carried out.The Plan of anesthesia induction and maintenance of anesthesia was the same for both grou Ps.After induction of anesthesia and before surgery,an ultrasound-guided Transverse Abdominis Plane(TAP)block is Performed.The Anaesthetic technique was standardized in both grou Ps.Grou P B received bilateral TAP block with 20 m L of 0.25% bu Pivacaine on each side whereas Grou P BM received bilateral TAP block with 20 m L of 0.25%bu Pivacaine with 150 mg of magnesium sulfate on each side.All Patients received the same medication,dose,and injection Parameters of PCIA Pum Ps.Data collected were,the time of the first com Pression on the PCIA Pum P,the number of effective analgesia Pum P com Pressions 24 h after o Peration,additional analgesics(hydromor Phone 1mg)times in the 24 hours after the o Peration,Posto Perative adverse events(such as nausea,vomiting,hy Potension,etc.),the satisfaction scores of the Patient,the Ramsay scores,rest and coμgh VAS scores,and Patient’s vitals at 2,4,6,12 and 24 hours after the o Peration.Result:1.The time of the first com Pression of the PCIA Pum P of Grou P BM was significantly Prolonged than Grou P B(P <0.05)2.The number of effective analgesia Pum P com Pressions in the 24 h hours after the o Peration of Grou P BM was significantly lower than Grou P B(P < 0.05)3.The times of additional analgesics in 24 h after o Peration of Grou P BM was significantly less than Grou P B(P<0.05)4.The VAS scores at rest and coμgh of Grou P BM were significantly lower than Grou P B(P<0.05)5.The Ramsay scores at 6h of Grou P BM were significantly higher than Grou P B(P<0.05)6.The satisfaction scores of the Patient of Grou P BM were significantly better than Grou P B(P<0.05).7.No significant difference was found in the incidence of nausea and vomiting in the two grou Ps.Conclusion : Bu Pivacaine with magnesium sulfate in USG guided Tranverse Abdominis Plane(TAP)block in La Parosco Pic Gynecological surgery under general anesthesia results in lower Posto Perative VAS scores,longer duration of analgesia,and less Posto Perative o Pioid requirement with no difference in the incidence of nausea and vomiting com Pared with bu Pivacaine alone.
