| Objective:To explore the clinical significance of peripheral blood lymphocyte / monocyte ratio(Lymphocyte to monocyte ratio,LMR),C-reactive protein / albumin ratio(C-reactive protein to albumin ratio,CRP/ALB),erythrocyte distribution width(Red Cell volume Distribution Width,RDW)and mean erythrocyte volume(mean corpuscular volume,MCV)in predicting children with HSP.Methods: A total of 100 Korean and Han children with HSP diagnosed in the Department of Pediatrics of the affiliated Hospital of Yanbian University from January 2010 to December 2021 were collected,and the healthy children of Korean and Han nationality in the same age group were selected as the control group.Statistics were made on the season,age,sex and the indexes of blood routine examination for the first time after admission,and LMR,CRP/ALB,RDW and MCV were calculated.SPSS26.0 statistical software was us ed for data analysis.The normality test results of all the data do not obey the normal distribution,so the quartile spacing is used to describe the characteristics of the data.Man Whitney U test was used to compare between groups,binary Logistic regression analysis was used to calculate the joint predictive factors,and the ROC curve of each variable was constructed,and the curve characteristics,predictive value,specificity and sensitivity were analyzed.The difference was statistically significant(P < 0.05).Results:1.Compared with the control group,the incidence of HSP in female was higher than that in male,and there was no significant difference between Korean and Han nationality in terms of sex,but it was divided into 5 groups according to age: 0-3 years old,3-6 years old,6-9 years old,9-12 years old,12-15 years old,and there was no significant difference between Korean nationality and Han nationality in each age group(P > 0.05).2.Seasonal morbidity: 41% in spring,14% in summer,22% in autumn,and 23% in winter.The incidence of Han nationality is 33% in spring,18% in summer,28% in autumn and 21% in winter.There was no significant difference between the Korean nationality and the Han nationality in spring and winter,and the difference between the two groups was not statistically significant(P > 0.05).3.The level of LMR in the Henoch-Schonlein purpura group was lower than that in the control group,while the levels of CRP/ALB and RDW in the anaphylactoid purpura group were significantly higher than those in the control group.There was no significant difference in the level of MCV between the control group and the control group.4.The ROC curve analysis of CRP/ALB,LMR,RDW,MCV and CRP/ALB+LMR joint prediction HSP: C RP/ALB: AUC=0.77,95%CI=0.734-0.82510 LMR: AUC=0.769,95%CI=0.722-0.816 scarf AUCPLOR 0.563 pr 95% CIIC 0.507-0.620 th MCVZ AUCZ 0.50 1 min 95% CIQ 0.444-0.558 Trans CRPBank ALBR LMR: AUC=0.843,95%CI=0.805-0.882.The maximum Youden index,YI of CRP/ALB+LMR is 0.474,and when CRP/ALB+LMR > 0.474,HSP may occur with a sensitivity of 73% and specificity of 84.5%.When CRP/ALB > 0.050,HSP may occur with a sensitivity of 62.5% and a specificity of 83.5%.The Yoden index of LMR is 0.460,and the cutoff value is 5.6 24.When LMR < 5.624,HSP may occur,the sensitivity is 67.00%,and the specificity is 79.00%.The Yoden index of RDW is 0.165,and the cut-off value is 38.250.When RDW > 38.250,HSP may occur,the sensitivity is 43.50%,and the specificity is 73%.In com parison,the sensitivity and specificity of joint prediction of CRP/ALB+LMR is the highest.5.There was significant difference in LMR,CRP/ALB and RDW between Korean HSP children and control group,but only LMR and CRP/ALB in Han HSP children had significant difference with control group,but RDW had no significant difference.There was significant difference in CRP/ALB between the Korean nationality and the Han nationality.Conclusion:1.CRP/ALB+LMR,CRP/ALB,LMR and RDW have certain predictive value for HSP,and the combined predictive value of CRP/ALB+LMR is the highest.2.There is a difference in the expression of RDW between Korean and Han children with HSP,which is related to genes and living habits. |
