Purpose:In clinical practice,it is observed that many adverse reactions of hand-foot syndrome and oral mucositis in breast cancer patients treated with pegylated liposomal doxorubicin.Through retrospective analyzed of the clinical data of breast cancer patients treated with pegylated liposomal doxorubicin,and discussed the related risk factors of hand-foot syndrome,oral mucositis,hand-foot syndrome and oral mucositis,so as provide reference for clinical prevention and treatment.Method:A total of 395 patients with breast cancer who received pegylated liposomal doxorubicin chemotherapy from October 2019 to December 2021 in a breast diagnosis and treatment center of a top three hospital in Qingdao were selected.The basic information and clinical data of patients were collected,and the adverse reactions and incidence of adverse reactions after using pegylated liposomal doxorubicin were sorted out.At the same time,the baseline and clinical characteristics were compared according to whether the patients had hand-foot syndrome,oral mucositis,hand-foot syndrome and oral mucositis,and the related risk factors of hand-foot syndrome,oral mucositis,hand-foot syndrome and oral mucositis were discussed by multivariate Logistic regression analysis.Result:1 Incidence of adverse reactions of pegylated liposomal doxorubicin1.1 In this study,the adverse reactions of 395 breast cancer patients treated with pegylated liposomal doxorubicin were summarized,which mainly involved skin system,gastrointestinal system,respiratory system,nervous system,blood system,systemic system,heart function,eye function and liver function,etc.1.2 There were 137 patients with hand-foot syndrome,with an incidence of 34.7%;there were 55 patients with oral mucositis,with an incidence of 13.9%;there were 46 patients with hand-foot syndrome and oral mucositis,with an incidence of 11.6%. Among them,hand-foot syndrome or oral mucositis is mainly grade I ~ II,and most of them occur 2 ~ 3 cycles after chemotherapy.2 Analysis of risk factors of hand-foot syndrome2.1 Univariate analysis of influencing factors of hand-foot syndrome: dose intensity,history of gallstones,liver cyst,ALT,AST,GGT and Hb were statistically significant with hand-foot syndrome(P < 0.05).2.2 Multivariate analysis of influencing factors of hand-foot syndrome: Through multivariate Logistic regression analysis,the results showed that dose intensity(P=0.020),history of gallstones(P=0.037),ALT(P=0.035),AST(P=0.016)and Hb(P=0.001)were related risk factors of hand-foot syndrome(P < 0.05).3 Analysis of risk factors of oral mucositis3.1 Univariate analysis of influencing factors of oral mucositis: dose intensity,ALT,AST,WBC and Hb were statistically significant with oral mucositis(P < 0.05).3.2 Multivariate analysis of influencing factors of oral mucositis: Through multivariate Logistic regression analysis,the results showed that the dose intensity(P=0.004)and WBC(P=0.009)were related risk factors of oral mucositis(P < 0.05).4 Analysis of risk factors of hand-foot syndrome and oral mucositis4.1 Univariate analysis of influencing factors of hand-foot syndrome and oral mucositis: dose intensity,molecular typing,history of gallstones,ALT,AST,WBC and Hb were statistically significant with co-occurrence of hand-foot syndrome and oral mucositis(P < 0.05).4.2 Multivariate analysis of influencing factors of hand-foot syndrome and oral mucositis: Through multivariate Logistic regression analysis,the results showed that the dose intensity(P=0.014),gallstone history(P=0.019)and WBC(P=0.009)were the related risk factors of hand-foot syndrome and oral mucositis(P < 0.05).Conclusion:In the retrospective study of pegylated liposomal doxorubicin in the treatment of breast cancer,it was found that the incidence of hand-foot syndrome and oral mucositis was high,and there were more patients with both of them.Multivariate Logistic regression analysis showed that dose intensity,history of gallstones,ALT,AST and Hb were the related risk factors of hand-foot syndrome;dose intensity and WBC were the related risk factors of oral mucositis;dose intensity,history of gallstones and WBC were the related risk factors for hand-foot syndrome and oral mucositis.In clinical practice,we should pay close attention to patients with the related risk factors of hand-foot syndrome or oral mucositis,so as to provide reference for the individualized treatment of such patients.
