Revision And Application Of The Scale Of Knowledge,attitude And Ability To Deal With Elder Abuse Of Medical Staff

Objectives:To provide a reliable evaluation tool for related research,revise the scale of knowledge,attitude and handling ability of elder abuse and conduct reliability and validity test;The revised scale was used to investigate the status of knowledge,attitude and ability to deal with elder abuse among medical staff and analyze the influencing factors,so as to provide theoretical basis for relevant training and policy making.Methods:1.Based on the version of knowledge,attitude and handling ability of elder abuse after cultural adjustment,a questionnaire survey was conducted among 839 conveniently selected medical staff.Through item analysis and reliability and validity test,the revised scale of knowledge,attitude and handling ability of elder abuse of medical staff was formed.2.Convenient to choose medical institutions at all levels of the questionnaire survey to 595 medical workers.The research tools were the general information questionnaire and the revised geriatric maltreatment knowledge,attitude and coping ability questionnaire.SPSS23.0 was used for data analysis.Statistical methods include descriptive analysis,independent sample t test,Pearson correlation,multiple linear regression.Results:1.The scale of knowledge,attitude and coping ability of elder abuse,revised by item analysis and exploratory factor,consists of three subscales,and the scale of knowledge consists of two dimensions and 26 items;The attitude scale includes 2 dimensions and 15 items.The scale of processing ability consists of 33 items in 3 dimensions.The K-R coefficient of knowledge scale was 0.935,the Cronbach’s a coefficient of attitude scale was 0.953,the Cronbach’s a coefficient of processing ability scale was 0.977.the Cronbach’s a coefficients of each dimension range from 0.903 to 0.970.The semi-reliability of knowledge scale was 0.682,the attitude scale was 0.869,and the processing ability scale was 0.924.The half-reliability of each dimension is between 0.867 and 0.943.2.The fitting effect of the scale after confirmatory factor analysis:the fitting indexes of the revised knowledge scale were χ2/df=3.040,RMSEA=0.059,NFI=0.900,CFI=0.931,IFI=0.931,PNFI=0.809,PCFI=0.836;The fitting indexes of attitude scale were χ2/df=4.841,RMSEA=0.080,NFI=0.950,CFI=0.960,IFI=0.960,PNFI=0.778,PCFI=0.786.The fitting indexes of handling ability scale were χ2/df=4.435,RMSEA=0.076,NFI=0.901,CFI=0.922,IFI=0.922,PNFI=0.825,PCFI=0.843.The model fitting effect is better than the cultureadjusted version.3.The average score on the scale of knowledge,attitude and handling ability of medical staff on elder abuse was(160.28±34.84),and the average score of the scale of knowledge,attitude and handling ability was(11.77±7.59),(41.62±10.12)and(106.89±27.98),respectively.The scores on the scale of knowledge and handling ability were relatively low.Pearson correlation analysis results showed that there was a positive correlation between the scores of knowledge,attitude and handling ability of elder abuse among medical staff(r=0.161,0.254,0.252,P<0.001).4.The results of multiple linear regression analysis showed that the gender of medical staff,whether they had found elder abuse in their work,whether they had dealt with elder abuse in their work and whether they had been exposed to relevant courses in school were the influencing factors of elder abuse knowledge(P<0.05);The influencing factors for elder maltreatment attitude were the type of work organization and understanding of the definition of elder maltreatment(P<0.05).The influencing factors of elder abuse handling ability were whether they had received relevant courses in school,type of working institution,knowledge level and attitude toward elder abuse(P<0.05).Conclusions:1.The revised scale of knowledge,attitude and ability to deal with elder abuse of medical staff has high reliability and validity,and can be used as a tool to evaluate knowledge,attitude and ability to deal with elder abuse of medical staff in mainland China.2.The education and training of medical staff on elder abuse is less,and the score of knowledge and processing ability is relatively low.It is necessary to strengthen relevant training and improve the system,policy and other measures to improve the knowledge and processing ability of medical staff on elder abuse.3.There is a positive correlation between knowledge,attitude and ability to deal with elder abuse.The level of knowledge of elder abuse is higher among female,health care workers who have previous experience in detecting or dealing with elder abuse and those who have received relevant courses in school.The medical staff who worked in primary health care institutions and had a good grasp of the definition of elder maltreatment had a more positive attitude towards elder maltreatment.The medical staff who had received relevant courses,higher knowledge and attitude score had better ability to deal with elder abuse.
