Objective To explore the correlation between MTHFRC677T gene polymorphism and serum homocysteine and folic acid levels in patients with type H hypertension and type Ⅱdiabetes,and to provide theoretical basis for early intervention of these two diseases.Methods The outpatients and inpatients in Zhejiang People’s Hospital from June 2020 to August 2021 were selected as the research objects,and the healthy people were selected as the control group,including 348 cases of H-type hypertension,511 cases of non-H-type hypertension,128 cases of type Ⅱ diabetes and 195 cases of healthy control group.The levels of serum homocysteine and folic acid were measured by chemiluminescence microparticle immunoassay.According to the difference of serum Hcy level,hypertensive patients were divided into H-type hypertension group and non-H-type hypertension group.By p Cr amplification of MTHFR C677T gene,the distribution characteristics of MTHFR C677T gene polymorphism among the target population were studied and genotyped.SPSS 22.0 statistical software was used for data analysis.Results 1.Among 859 subjects,the genotype frequencies of CC,CT and TT in H-type hypertension group were 34.2%,47.7%and 18.1%respectively,while those in non-H-type hypertension group were 42.3%,47.2%and 10.5%respectively,and the difference of genotype frequencies between the two groups was statistically significant(p<0.05 The frequency of T allele at MTHFR C677T locus in type H hypertension group(42.0%)was higher than that in non-type H hypertension group(34.1%),and the difference was statistically significant(p<0.05).2.Mutation of 2.MTHFR C677T gene is an independent genetic risk factor for H-type hypertension(p<0.05).Compared with CC genotype,the risk of H-type hypertension is higher in CT and TT genotype(OR=6.132,95%CI:2.457-15.681,p<0.05;OR=3.082,95%CL:1.663-5.847,p<0.05)。3.The polymorphism of MTHFR C677T gene in patients with H-type hypertension and type Ⅱ diabetes mellitus was statistically significant(p<0.05).The serum Hey level of TT genotype was higher than that of CT genotype,and that of CT genotype was higher than that of CC genotype(R=0.642,p<0.05).The plasma folate level of TT genotype was lower than that of CT genotype,and that of CT genotype was lower than that of CC gene(R=-0.50 4.The serum Hcy level of patients with type H hypertension is different among different hypertension grades(p<0.05),and with the increase of the risk degree of hypertension,its serum level also increases(R=0.684,p<0.05).5.There is a negative correlation between serum Hcy level and folic acid level in patients with type H hypertension and type Ⅱ diabetes(r=-0.498,p<0.05).Conclusion 1.Mutation of MTHFR C677T gene is a susceptible genetic factor for the occurrence of H-type hypertension and type Ⅱ diabetes mellitus.2.Mutation of MTHFR C677T gene can lead to an increase in serum homocysteine level and a decrease in serum folate level.Low folate level and MTHFR C677T gene polymorphism may increase the incidence of H-type hypertension and type Ⅱ diabetes mellitus in.people.3.There is a positive correlation between blood pressure classification and serum Hcy level in patients with H-type hypertension,suggesting that there may be a certain correlation between the increased risk of hypertension and the increased Hcy level,and it is necessary for patients with H-type hypertension to intervene Hcy level while controlling blood pressure.4.The serum Hcy of patients with type H hypertension and type Ⅱ diabetes increased with the decrease of folic acid level.Folic acid is a safe and effective supplement,which can effectively reduce the blood Hcy level of patients with type H hypertension.In this study,the serum Hcy of patients with type H hypertension and type Ⅱ diabetes with TT MTHFR C677T gene is higher and folic acid level is lower.Therefore,early intervention and appropriate preventive folic acid supplementation may bring benefits to these people.
