The Effect Of I.lanceolatum Injection For Relieving Pain And Swelling After Extraction Of Impacted Mandibular Third Molar

ObjectivePatients were often accompanied by adverse reactions such as pain,swelling and trismus,which affected the quality of life of patients after extraction of mandibular third molar.The purpose of this study was to evaluate postoperative pain,swelling and trismus after usage of i.lanceolatum injection,and to provide alternative for drugs alleviating postoperative adverse reactions after extraction of mandibular third molar.Methods(1)Ninety adult patients were recruited in The First Affiliated Hospital of Zhengzhou University,Daxue Road Dental Clinic for impacted lower third molar extraction.According to the random number table,they were assigned to the i.lanceolatum group,the dexamethasone group(positive control group)and the blank group(blank control group),with 30 cases in each group.(2)Patients didn’t receive any injection in the blank group after surgery,patients in the i.lanceolatum group received i.lanceolatum injection,and dexamethasone injection was adopted in the dexamethasone group.Postoperative pain.swelling,and trismus were recorded at day 1,day 3,and day 7.The satisfaction was recorded on the seventh day.follow-up was performed at day 1,day 3 and day 7 after impacted lower third molar extraction.The degree of swelling was evaluated by facial swelling grade,the pain degree and the patient satisfaction were measured by visual analog scale,the reduction in mouth opening was used to assess trismus,and the healing grade was used to evaluate the degree of postoperative swelling.(3)The pain,swelling,trismus,healing and patient satisfaction among the three groups were compared at day 1,day 3 and day 7.The date was analyzed by SPSS version 25.0,significant level α=0.05,the P<0.05 was considered significant.Results(1)The pain.At day 1,the VAS of patient pain in the blank group was higher than that in the i.lanceolatum group(P<0.05).There was no statistical significance between the i.lanceolatum group and the dexamethasone group(P>0.05);At day 3,the VAS of the i.lanceolatum group was lower than that in the blank group(P<0.05).There was no statistical significance between the i.lanceolatum group and the dexamethasone group(P>0.05);There was no statistical significance among the three groups at 7 days after surgery(P>0.05).(2)The swelling.The swelling of the i.lanceolatum group was lower than that in the blank group at day 1 and day 3 after impacted lower third molars extraction(P<0.05).There was no statistical significance between the i.lanceolatum group and the dexamethasone group(P>0.05).There was no significant difference in the degree of swelling among the three groups at day 7(P>0.05).(3)The trismus.At day 1,the trismus in the i.lanceolatum group was lower than that in the blank group(P<0.05).There was no statistical significance between the i.lanceolatum group and the dexamethasone group(P>0.05);At day 3,the trismus of the i.lanceolatum group was lower than that in the blank group(P<0.05).There was no statistical significance between the i.lanceolatum group and the dexamethasone group(P>0.05);There was no statistical significance among the three groups at day 7 after surgery(P>0.05).(4)The patient satisfaction and healing.At day 7,the patient satisfaction in the i.lanceolatum group was(9.19±0.63),which was higher than that in the blank group(P<0.05).There was no statistical significance between the i.lanceolatum group and the dexamethasone group(P>0.05).There was no poor wound healing in the three groups of patients after surgery,all of which were grade A healing.ConclusionThe adverse reactions,including postoperative pain,swelling,and trismus in mandibular third molar extraction,could be released by using i.lanceolatum injection.The patient satisfaction can be improved effectively.There was no negative impact on wound healing after using i.lanceolatum injection.
